Abstract


RÉSUMÉ
Introduction. Les accidents vasculaires cérébraux constituent un problème de santé publique à travers le monde. L’objectif de notre étude était de décrire les accidents vasculaires cérébraux au service de médecine de l’Hôpital Sominé Dolo de Mopti. Méthodes. Nous avons effectué une étude transversale descriptive rétrospective couvrant la période de janvier à décembre 2016. Ont été inclus dans l’étude tous les patients hospitalisés durant cette période pour accident vasculaire cérébral. Résultats. Au total, nous avons colligé 134 patients sur 468 patients, soit une prévalence hospitalière de 28,63 %. Il y avait 56,72 % d’hommes et 70,15% des patients avaient plus de 60 ans. Les facteurs de risque les plus fréquents étaient  l’hypertension artérielle (42,54 %), le diabète (13,43 %) et l’hypercholestérolémie (8,95%). L’hémiplégie a été le déficit neurologique le plus fréquent (34,33%). Les accidents vasculaires cérébraux ischémiques ont représenté 55,22% des cas. Parmi ces patients, 67,16% ont été mis sous anti hypertenseurs. L’évolution a été satisfaisante chez 83% des patients. Le taux de létalité globale a été de 17%. Conclusion. La présentation des AVC dans notre service suit les schémas habituels. La prévention des accidents vasculaires cérébraux constitue la mesure la plus efficace dans notre milieu à travers une meilleure prise en charge des facteurs de risque cardiovasculaires, la mise en place d’une unité neurovasculaire.
ABSTRACT
Introduction. The objective of the study was to describe the clinical profile of patients with stroke admitted in a sub-Saharan African setting. Methods. This was a cross sectional descriptive retrospective study that covered the period from January 2016 to December 2016. All the patients hospitalized for stroke during that period in the department of internal medicine of Sominé Dolo hospital of Mopti were recruited. The diagnosis of stroke was based of clinical presentation and CT findings. Our data of interest were sociodemographic data, clinical presentation, type of stroke and outcome. Results. We recruited 134 patients out of 468 patients, giving a hospital prevalence of 28.63%. Male patients represented 56.72% of the series. Patients aged 60 years or more  represented 70.15% of the series. The most frequent risk factors were high blood pressure (42.54%), diabetes (13.43%) and hypercholesterolemia (8.95%). Hemiplegia was the most frequent neurological deficiency (34.33%). Ischemic stroke represented 55.22% of the cases. 67.16% of patients were ordered blood pressure medications The evolution was satisfactory for 83% of the patients. The lethality rate was 17%. Conclusion. Clinical presentation of stroke in our department follows the common pattern. The prevention of cerebrovascular accidents is the most effective step in our area through a better treatment of cardiovascular risk factors, the establishment of a neurovascular unit.
