Abstract

RÉSUMÉ
Introduction. La drépanocytose est l’hémoglobinopathie structurale la plus fréquente. Elle est causée par la production d’une hémoglobine anormale « HbS » sur le chromosome 11. Cette mutation se traduit par la substitution de l’acide glutamique par de la valine en sixième position de la chaîne béta. Le but de l’étude était de décrire les manifestations uroandrologiques de la drépanocytose au service d’urologie du Centre Hospitalier Universitaire de Référence Nationale de Ndjamena. Patients et méthodes. Il s’agit d’une étude rétrospective descriptive colligeant les dossiers des patients souffrant de complications uroandrologiques de la drépanocytose de janvier 2011 à décembre 2018. Etaient inclus les patients de deux sexes, âgés de 15 ans et plus dont le diagnostic de la drépanocytose a été confirmé par l’électrophorèse de l’hémoglobine et suivis pour priapisme ou hématurie. Les variables étudiées étaient sociodémographiques, cliniques, paracliniques, thérapeutiques et évolutives. Résultats. Au total, treize dossiers de patients étaient éligibles sur un ensemble de 3050 soit 0,41%. L’âge moyen des patients était de 26,38 ans. Le sex-ratio était de 5,5. Les arabes étaient les plus affectés (46,1%). Les motifs d’hospitalisation étaient le priapisme (76,9%) et ou l’hématurie (23,1%). Le traitement était médical (100%) et/ou chirurgical (61,5%). L’évolution des patients était favorable (84,6%). Les complications étaient dominées par l’insuffisance rénale (15,4%). La durée moyenne d’hospitalisation était de 13 jours. Conclusion. les principales manifestations uro-andrologiques de la drépanocytose étaient le priapisme et l’hématurie. La prise en charge est médicale et chirurgicale.
ABSTRACT
Introduction. Sickle cell disease is the most common structural hemoglobinopathy. It is caused by the production of an abnormal hemoglobin "HbS" on chromosome 11. This mutation results in the substitution of glutamic acid by valine in the sixth position of the beta chain. The aim of our study was to describe the uroandrological manifestations of sickle cell disease in the urology department of the Centre Hospitalier Universitaire de Référence Nationale of Ndjamena. Patients and methods. This was a retrospective transversal descriptive study collecting the records of patients suffering from uro-andrological complications of sickle cell disease from January 2011 to December 2018. We included patients of both sexes, aged 15 years and older, whose diagnosis of sickle cell disease was confirmed by hemoglobin electrophoresis and who were followed for priapism or hematuria. The variables of interest were: sociodemographic data, clinical presentation, paraclinical findings, treatment and outcome. Results. Thirteen patients (0.41%) were eligible out of a total of 3050. Their mean age was 26.38 years. The sex ratio was 5.5. Arabs were mostly affected (46.1%). The reasons for hospitalization were priapism (76.9%) and/or hematuria (23.1%). Treatment was medical (100%) and/or surgical (61.5%). The evolution of the patients was favorable (84.6%). The main complications was renal failure (15.4%). The average length of hospitalization was 13 days. Conclusion. The main uroandrological manifestations of sickle cell disease are priapism and hematuria. The management is medical and surgical.
