Abstract

RÉSUMÉ
Objectif. Préciser l’apport de la biopsie per-cutanée guidée par scanner dans les formes douteuses de lésions disco vertébrales. Patients et méthodes. Il s’est agi d’une étude descriptive allant de janvier à août 2018 dans les services de chirurgie polyvalente, de rhumatologie et d’imagerie médicale du CHU de Brazzaville. Nous avons inclus tous les patients présentant des lésions disco vertébrales ayant bénéficiés d’un scanner et/ou une IRM n’ayant pas permis un diagnostic lésionnel de certitude et pour lesquelles une indication de biopsie à ciel ouvert avait été indiquée. Toutes les biopsies ont été guidées par scanner et réalisées à l’aide d’une aiguille coaxiale de marque BIOMEDICAL Sarl. Résultats. Au total, 27 patients ont bénéficié d’une PBDV. L’âge moyen des patients était de 48,3 ans (30 – 72 ans) avec un sexe ratio 1,25. La topographie lésionnelle était lombaire dans 55% cas puis dorsale dans 33% des cas. Trois prélèvements étaient non contributifs. La PBDV était contributive au diagnostic chez 24 patients (89 %). Elle avait permis le diagnostic de processus infectieux évolutif chez 14 patients avec isolement du Bacille Koch chez 12 d’entre eux. Les lésions tumorales étaient dominées par les localisations secondaires. Nous avons noté un seul cas de douleur intense ayant nécessité l’interruption du geste. Conclusion.: La PBDV guidée par scanner est une technique efficace, peu invasive, à coût modéré. Elle donne une évaluation rapide et souvent décisive de l'étiologie néoplasique ou infectieuse de lésion disco vertébrale.


ABSTRACT
Objective. To clarify the contribution of the CT-guided percutaneous biopsy in doubtful forms of disco-vertebral lesions. Patients and methods. We conducted a descriptive study from January to August 2018 in the departments of multidisciplinary surgery, rheumatology and medical imaging at the University Hospital of Brazzaville. We included patients with disco-vertebral lesions who had undergone a CT scan and / or an MRI that did not allow a confirmed lesional diagnosis and for whom a chirurgical biopsy had been indicated. All biopsies were guided by CT-scan and performed using a BIOMEDICAL Sarl brand coaxial needle. Results. A total of 27 patients received a CT-guided DVB. The average age was 48.3 years (30 - 72 years) with a sex ratio of 1.25. The lesion topography was lumbar in 55% of cases then dorsal in 33% of cases. Three biopsies were non-contributory. CT-guided DVB helped make the diagnosis in 24 patients (89%). It enabled the diagnosis of an evolving infectious process in 14 patients with isolation of Bacillus Koch in 12 of them. Tumor lesions were dominated by secondary locations. We noted only one case of severe pain requiring the interruption of the procedure. Conclusion. CT-guided DVB is an efficient, minimally invasive, low-cost technique. It provides a rapid and often decisive assessment of the neoplastic or infectious etiology of disco-vertebral lesion.
