Abstract

ABSTRACT

In Cameroon, there is an increased incidence of postoperative infection in reference hospitals. Few studies have been done on the postoperative morbidity and nature of complications in the district hospitals.

OBJECTIVE: The aim of this study was to determine the frequency and the clinical presentations of early postoperative complications in Yaoundé district hospitals and to search for the determinants.

PATIENTS AND METHODS: We carried out a prospective, analytical and multicentric study over a five-month period from October 1, 2013 to February 28, 2014. Patients aged 18 years and above admitted in the surgery units following surgical treatment were included. Follow-up of the patients was done through daily physical examination during rounds to register eventual complications.

RESULTS. We included 265 patients in our study; the mean age was 31.5 years with a  sex ratio of  1:7. Thirty-three (12.5%) patients presented 38 complications of which two (0.75%) were fatal. Overall frequency of complications was 14.3% (38/265). Postoperative infection was the leading complication 50.1% (19/265) followed by postpartum hemorrhage 34.2% (13/230), bowel obstruction 5.3% (2/265) and deaths 5.3% (2/265). These complications were favoured by emergency, a preoperative hospitalization lasting more than 7 days, abdominal obesity, more than 2 hours lasting procedures, an ASA score of III, Altemeier classes 3 and 4 and a NNISS score ≥ 2 (p value< 0.05).

CONCLUSION. The overall frequency of early postoperative complications in the Yaounde district hospitals is 14.3%. The main clinical presentation is infection.

RÉSUMÉ

Au Cameroun où  l’incidence des complications infectieuses postopératoires reste élevée dans les hôpitaux de référence, plusieurs zones d’ombre persistent sur la morbidité postopératoire dans les hôpitaux de district.

OBJECTIF : Déterminer la fréquence, les tableaux cliniques et  les déterminants des complications postopératoires précoces dans les hôpitaux de district de la ville de Yaoundé.

PATIENTS ET MÉTHODES : Il s’agit d’une étude prospective, analytique et multicentrique allant du 1er octobre 2013 au 28 février 2014. Les patients âgés d’au moins 18 ans, opérés et hospitalisés en chirurgie générale et gynéco-obstétrique et ayant présenté des complications pendant les 30 jours postopératoires ont été inclus. Les patients étaient examinés quotidiennement au cours de la visite à la recherche d’éventuelles complications.

RÉSULTATS : Nous avons colligé 265 patients avec un sex-ratio de 1 :7. L’âge moyen était de 31,5 ans. Trente-trois (12,5%) patients ont eu 38 complications au total, dont deux (0,75%) ont été mortelles. La fréquence globale des complications était de 14,3% (38/265). Les  complications étaient dominées par l’infection 50,1% (19/265) ; suivie par l’hémorragie du post-partum 34,2% (13/230), les occlusions intestinales 5,3% (2/265)  et les décès 5,3% (2/265).  Ces complications étaient favorisées par l’urgence, une durée d’hospitalisation préopératoire supérieure à 7 jours, l’obésité, une durée d’intervention supérieure à 2 heures, le score ASA III, les classes 3 et 4 d’Altemeier et le score de NNISS 2-3 (valeur p<0,05).

CONCLUSION : Les complications postopératoires sont fréquentes (14,3%) dans les hôpitaux de district de la ville de Yaoundé et dominées par l’infection.
