Abstract

Objectifs. L’infertilité occupe une place importante dans les consultations gynécologiques. Cela entraine un nombre important de demandes d’échographies dans notre structure qui est un hôpital de District. Dans cette étude, nous avons voulu recenser les lésions échographiques chez les femmes infertiles afin de mieux cerner la place de l’échographie dans le bilan des hypofertilités et infertilités féminines dans un contexte où des technologies concurrentes comme l’IRM pointent à l’horizon. Patients et méthodes. Il s’est agi d’une étude rétrospective de 4 ans qui a recensé tous les résultats échographiques des patientes venues réaliser une échographie pour bilan de fertilité, désir de grossesse et monitorage folliculaire. Les paramètres retenus pour l’analyse étaient l’âge des patientes, les prescripteurs, les résultats échographiques répertoriés en pathologie utérine, ovariennes et  tubaires. L’analyse des données à été faite avec le logiciel Epi Info 5.1 dans le strict respect de la confidentialité et de l’anonymat.  Résultats.  121 patientes ont été recensées. L’âge des patientes variait de 17 à 49 ans avec une moyenne de 31 ans. 87 pathologies ont été diagnostiquées à l’échographie. Il y avait  67.82% de  pathologies ovariennes ; 25.28 %  de pathologies utérines et  6.89 %  de pathologies tubaires. Le syndrome des ovaires polykystique (SOPK) était la pathologie majoritaire dans les étiologies ovariennes.  Il existait des pathologies associées telles que les synéchies et les adénomyoses dans une proportion respectivement de 1,14 % et 3,44  %. Conclusion. L’échographie a été d’une utilité certaine en permettant le diagnostic des pathologies ovariennes  et le suivi thérapeutique des inductions de l’ovulation par le monitoring. Elle a  montré ses limites dans l’étude de la pathologie tubaire.
ABSTRACT
Infertility is a frequent problem in gynecologic consultations. For this reason, we have a significant number of ultrasound requests in our District Hospital. The purpose of our study was to report the ultrasonographic findings in infertile patients and assess the role of ultrasound when other tools such as MRI are emerging.
Patients and methods. Tthis was a retrospective study of 4 years that identified all the ultrasound results from patients who achieved a sonogram for infertility workup, desire for conception or follicular monitoring. The parameters used in the analysis were the age of patients, prescribers, sonographic findings grouped in uterine, ovary, and tubal lesions. Analysis of data was done with software Epi Info 5.1 in strict confidentiality and anonymity.
Results: 121 patients were recruited. The age of patients ranged from 17 to 49 years with an average of 31 years. 87 pathologies were diagnosed. There were ovarian in 67.82% of cases; uterine in 25.28% and tubal in 6.89% cases. Polycystic ovarian syndrome was the main ovarian pathology. Some other lesions were described such as adhesions and the adenomyose in respectively of 1.14% and 3.44% of cases.
Conclusion: Ultrasound was useful essentially in ovarian disease diagnosis and monitoring of ovulation. It showed s its limits in the study of tubal pathology.
