Abstract

Introduction : Les tumeurs de la glande parotide sont rares et elles sont dominées par l’adénome pléomorphe. Le tableau clinique habituel est celui d’une tumeur indolore de a région parotidienne. La parotidectomie avec analyse histologique est nécessaire pour caractériser la lésion. Le but de ce travail était d’établir un bilan de la chirurgie parotidienne pratiquée à l’Hôpital Général de Yaoundé sur une période de dix ans.
Patients et méthodes. Il s’agit d’une étude rétrospective portant sur 15 cas colligés de 2005-2015. Nous avons sélectionné les dossiers des patients qui avaient une tuméfaction parotidienne, qui avaient été opérés, et avaient des résultats anatomopathologiques. Pour ces cas, nous avons collecté les données épidémiologiques, cliniques, paracliniques, thérapeutiques, évolutives et les avons comparées aux données de la littérature.
Résultats. L’âge moyen était de 33.53 ans. Le mode de révélation était dans tous les cas une tuméfaction de la région parotidienne. L'échographie a été effectuée dans 66,66% et la tomodensitométrie dans 13,33% des cas. une cytoponction a été réalisée chez 40% des patients, et s’est révélée négative dans tous les cas. Le type histologique prédominant était l’adénome pléomorphe (78,6%). La parotidectomie totale a été pratiquée dans 86.66% des cas. Dans les suites opératoires, une paralysie faciale périphérique a été notée dans 13.34 % des cas et la récidive tumorale dans 6.6% des cas.
Conclusion : les tumeurs parotidiennes sont rares. La cytoponction à l'aiguille fine est peu contributive au diagnostic et retarde parfois l'exérèse chirurgicale. La parotidectomie totale est recommandée dans notre contexte.

ABSTRACT
Introduction: parotid gland tumors are rare. Pleomorphic adenomas are predominant and usually present as a swelling of the parotid region. Parotidectomy with histopathology analysis is required to affirm the nature of the lesion. The aim of the study was to analyze parotid gland surgeries realized over a period of 10 years at Yaounde General Hospital.
Methods: this was a retrospective study comprising 15 cases managed over a period of 10 years from 2005 to 2015, concerning patients operated for parotid gland tumors. The following data were collected and compared to the published literature: sociodemographic, clinical, paraclinical, therapeutic and evolutive.
Results: the mean age was 33.53 years. All patients presented with swelling of a parotid region. Cervical ultrasound was done on 66.66% patients and computed tomography in 13.33% of cases. Fine needle aspiration was done in 40% of cases, and was always negative. Pleomorphic adenoma was the main histological finding. Total parotidectomy was done on 86.66% of patients. Long standing peripheral facial palsy was recorded in 13.34% of cases and tumor recurrence in 6.6% of cases.
Conclusion: parotid gland tumors are rare. Fine needle aspiration is rarely contributive. Total parotidectomy is the treatment of choice.
