Abstract

Objectif. L’objectif de notre étude était de montrer la contribution du repérage par harpon dans le traitement chirurgical des lésions mammaires non palpables au Burkina Faso.
Méthodologie. Une étude transversale descriptive a été réalisée chez 09 patientes, présentant 10 lésions mammaires infra-cliniques sur une période d’un an, dans une structure sanitaire privée de la ville d’Ouagadougou, après la mise en place d’un harpon sous contrôle échographique.
Résultats. Le dépistage systématique par mammographie et échographie constituait le moyen de découverte de ces nodules. L’âge médian était de 42 ans. La taille médiane des nodules était de 14,5 mm. La tumorectomie a été effectuée chez toutes les patientes en marge saine à l’exception d’une seule patiente qui a bénéficié d’une reprise chirurgicale. On retrouvait à l’histologie 3 lésions malignes, 5 bénignes et 2 lésions frontières.
Conclusion. Le harpon guide le geste chirurgical dans l’exérèse des lésions infra-cliniques du sein, permettant de concilier rôle carcinologique et esthétique.

ABSTRACT
Objectives. The aim of our study was to show the contribution of the harpoon technique in the surgical treatment of non-palpable breast lesions in Burkina Faso.
Methods. A descriptive cross-sectional study was performed in 09 patients, with 10 non-palpable breast lesions over a period of one year, in a private health facility in the city of Ouagadougou, after insertion of a harpoon-shaped metallic thread under ultrasound guidance.
Results. Routine screening mammography and ultrasound was the way to discovery of these nodules. The median age was 42 years. The median size of the nodules was 14.5 mm. Lumpectomy was performed in all patients in healthy margin with the exception of one patient who underwent surgical recovery. We found 3 malignant lesions, 5 benign lesions and 2 lesions bordelines.
Conclusion. The harpoon guides the surgical resection in non palpable breast lesions and limits excision of breast anomalies.
