Abstract

Introduction: Parmi les méthodes diagnostiques, le scanner (TDM) et l’imagerie par Résonnance Magnétique (IRM) jouent un rôle capital dans la prise en charge de la pathologie tumorale intracrânienne. Le but de notre étude était de déterminer le degré de concordance entre le diagnostic anatomopathologique et le diagnostic radiologique (TDM et IRM) des tumeurs intracrâniennes.
Matériels et méthodes: Il s’agit d’une étude rétrospective, transversale et descriptive menée dans les services de radiologie et d’imagerie médicale et d’anatomopathologie de l’Hôpital Général de Douala sur la période allant de janvier 2014 à mars 2015. Tous les patients admis pour suspicion de tumeur intracrânienne avec réalisation d’une TDM ou d’une d’IRM cérébrale étaient éligibles dans l’étude. Les données recueillies ont été saisies et analysées à l’aide des logiciels Microsoft 2013, SPSS 22.0 et Sphinx plus2 version 5.1.0.6 de 2012.
Résultats: Cent trente-trois patients ont été retenus. Le sex ratio H/F était de 1,46. L’âge moyen était de 39 ans. Parmi les examens réalisés, la prévalence des tumeurs intracrâniennes était de 5,60%. Le scanner cérébral a été réalisé chez 82 patients (61,70%) et l’IRM chez 51 patients (38,30%). Les diagnostics radiologiques les plus fréquemment évoqués étaient : tumeurs gliales (45,90%), adénomes hypophysaires (20,30%) et méningiomes (13,50%). Chez les enfants, les tumeurs les plus fréquentes étaient l’astrocytome et l’épendymome. L’analyse histologique a été obtenue chez 51 patients (38,30%). Le diagnostic radiologique a été confirmé chez 44 patients (86,30%). L’IRM était la modalité d’imagerie qui permettait le mieux de caractériser les lésions tumorales intracrâniennes.
Conclusion: L’association des données cliniques et de l’imagerie en coupe permet de manière fiable de poser un diagnostic concordant avec les données anatomopathologiques.


ABSTRACT
Background: CT scan and MRI play an essential role in the diagnosis of intracranial tumours. The aim of our study was to study the degree of concordance between radiological diagnosis with CT scan and MRI and pathological diagnosis of intracranial tumours.
Materials and methods: This was a transversal and descriptive study from January 2014 to March 2015. All patients admitted at the General Hospital of Douala for suspicion of intracranial tumour were eligible for the study.
Results: 133 patients were recruited, with a sex ratio of M/F of 1.46. The mean age was 39 years. The prevalence of intracranial tumours was 5.60%. CT scan was realized on 82 patients (61.70%) and MRI on 51 patients (38.30 %). The main radiological diagnoses were glial tumours (45.90%), hypophyseal adenoma (20.30%) and meningioma (13.50%). In the paediatrics population, astrocytoma and ependymoma were the tumours most frequently found. Histological diagnosis was available for 51 patients (38.30 %) and the radiological diagnostic was confirmed for 44 patients (86.30%). MRI was the modality with the best performance in terms of characterization of intracranial tumours.
Conclusion: Association of clinical and radiological data perform well in the diagnostic management of intracranial tumours.
