Abstract


RÉSUMÉ
Introduction. Le but de cette étude était de décrire la prise en charge chirurgicale des traumatismes du rachis thoraco-lombo-sacré au Centre Hospitalier Universitaire (CHU) de Brazzaville. Méthodologie. Nous avons mené une étude descriptive monocentrique de janvier 2014 à décembre 2015, au CHU de Brazzaville. Nous avons inclus tous les patients hospitalisés pour un traumatisme du rachis thoraco-lombo-sacré, puis exclus les cas avec données incomplètes. Les paramètres évalués étaient cliniques, radiologiques, chirurgicaux et évolutifs. Résultats. 25 patients ont été recrutés. L’âge moyen était de 37,8±15,3 ans et le sex ratio H/F était de 3,8. Les traumatismes étaient liés à un accident de la voie publique dans 66,66% des cas. À l’examen initial, 37,5% des patients étaient classés C sur l’échelle de l’American Spinal Injury Association (ASIA). Le délai moyen de réalisation du scanner était de cinq jours. Quinze patients ont été opérés, par abord postérieur avec réalisation d’une ostéosynthèse à visée pédiculaire, sous contrôle scopique, dans un délai moyen de 13 jours. La durée moyenne d’hospitalisation des patients était de 18,04±14,90 jours. La mortalité était de 8,33%. Conclusion. le traitement chirurgical des traumatismes du rachis thoraco-lombo-sacré est réalisé dans des délais qui pourraient être raccourcis, par un préfinancement des soins pour les patients victimes d’accidents de la voie publique qui constituent la majorité de notre série. Le développement des abords antérieurs du rachis thoraco-lombaire traumatique nécessite un perfectionnement des chirurgiens intéressés par cette pathologie.
ABSTRACT
Introduction. The aim of this study was to describe the management of traumatic thoraco-lombar spine and sacrum injuries in the University Hospital Center of Brazzaville. Methods. This was a cross sectional descriptive study from January 2014 to December 2015, in the University Hospital Center of Brazzaville. We included all patients admitted for thoraco-lombar and sacrum injuries. Our study parameters were clinical, radiologic, surgical and evolutive. Results. We recruited 25 patients. Their mean age was 37.8 ± 15.3 years and the sex ratio was 3.8. Spine injuries were due to road accident in 66.66% of cases. At the initial examination, 37.5% of patients were in the C class of American Spinal Injury Association (ASIA). The average delay for CT-scan was five days. Surgery was performed by posterior approach with pedicular osteosynthesis for fifteen patients, within an average of 13 days. Patients' hospital stay averaged 18.04 ± 14.90 days. The mortality was 8.33%. Conclusion. Timing of surgery can be improved by pre-financing of care for patients from road accidents which constitute most of our patients. There is a need to improve capacities of The development of anterior approach in the traumatic thoracolumbar spine requires an improvement of the surgeons interested by this pathology.
