Abstract


RÉSUMÉ
But. Décrire les aspects cliniques et la prise en charge chirurgicale des tumeurs gastriques à Dakar. Méthodologie : Il s’agit d’une étude rétrospective descriptive portant sur 36 patients suivis pour tumeur gastrique de janvier 2010 à décembre 2016. Nous avons inclus tous les patients qui avaient bénéficié d’un traitement chirurgical à visée curative ou palliative. Les variables étudiées étaient sociodémographiques, clinique, histopathologiques et thérapeutiques. Résultats. Les cancers gastriques représentaient 0,42% de l’activité opératoire. Il y avait 30 hommes et 6 femmes soit un sex ratio de 5/1. Leur âge moyen était de 58 ans [extrêmes 25 ans - 81 ans]. Le tabagisme chronique était retrouvé chez 6 patients (16%). Un long passé d’épigastralgies était noté chez 23 patients. Les symptômes étaient constitués d’amaigrissement dans 33 cas (91%), de signes de sténose du pylore dans 29 cas (79%), d’hématémèse dans 7 cas (20%), d’épigastralgies isolées dans 3 cas (8%) et de dysphagie dans 2 cas (5%). On notait une masse sus ombilicale dans 8 cas (23%) et une hépatomégalie tumorale associée dans 3 cas (8,8%). Une adénopathie sus claviculaire gauche était retrouvée dans 2 cas (5%). Le siège de la tumeur était antral dans 27 cas (79%). La tumeur était ulcérobourgeonnante dans 29 cas (85%) et infiltrante dans 4 cas (11%). L’histologie a décrit un adénocarcinome dans 32 cas (94%) et un lymphome MALT (mucosa associated lymphoid tissue) dans 1 cas (4%). Une gastrectomie des 4/5 avec curage de type D1,5 a été faite dans 11 cas et une gastrectomie totale dans 2 cas. Une tumeur localement avancée ou métastatique était présente en pré et peropératoire dans 23 cas (63%) ; dans ces cas, une dérivation ou gastrectomie a été réalisée. Nous avons eu en post opératoire deux décès précoces soit 5,8%. La survie à 2 ans était de 53% (7 cas) et 30% à 5 ans (4 cas). Conclusion. Le cancer affecte essentiellement l’homme adulte. Les formes précoces sont rares dans notre pratique. La réalisation plus facile de l’endoscopie pour des troubles dyspeptiques doit être encouragée.

ABSTRACT
Aim. To report the results of the surgical management of gastric cancer at Dakar. Materials and Methods. This was a retrospective descriptive study of patients treated for gastric cancer from January 2010 to December 2016. All patients who had undergone curative or palliative surgical treatment were included in the study, giving a total of 36 patients. Variables of interest were epidemiological, clinical and therapeutic. Results: Gastric cancer accounted for 0.42% of our surgical activity. There were 30 men and 6 women. Their mean age was 58 years [extremes 25 years - 81 years]. Chronic smoking was found in 6 patients (16%). Chronic epigastralgia was a complaint for 23 patients. Other clinical findings were weight loss in 33 cases (91%), signs of pyloric stenosis in 29 cases (79%), hematemesis in 7 cases (20%), dysphagia in 2 cases (5%), isolated epigastralgia in three cases (8%). A mass was present in clinical exam in 8 cases (23%) and associated hepatomegaly in three cases (8%). A left supraclavicular adenopathy was found in two patients. The site of the tumor was distal in 27 cases (79%). She was ulcerative in 29 cases (85%) and infiltrating in four cases (11%). Histology found adenocarcinoma in 32 cases (94%), lymphoma (mucosa associated lymphoid tissue) in one case (4%). Gastrectomy of 4/5 and type D1.5 lymphadenectomy were done in 11 cases, while total gastrectomy was done in two patients. A locally advanced or metastatic tumor was observed in pre and intraoperative in 23 cases (63%); for them, derivation or gastrectomy was performed. Two postoperative deaths were recorded (5.8%). Survival was 53% (7 patients) at two years and 30% (4 patients) at five years. Conclusion. Gastric cancer affects mainly adult men in Dakar. Early gastric cancer is rarely found. Prompt endoscopy for dyspeptic disorders or epigastralgia should be encouraged.
