Abstract

RÉSUMÉ
But : Décrire l'atteinte de la couche des fibres nerveuses rétiniennes (FNR) à la tomographie en cohérence optique (OCT) de la papille. Matériels et Méthodes: Il s'agit d'une étude prospective descriptive et analytique menée du 1er janvier au 31 décembre 2014 dans un cabinet privé d'ophtalmologie qui réalise l'OCT à Lomé. Les variables étudiées étaient l'âge, le sexe et les mensurations à l'OCT de la papille. Résultats: 335 examens réalisés chez 167 hommes et 168 femmes ont été retenus. L'OCT de la papille était indiquée pour exploration d’une grande excavation papillaire (GEP) ou une suspicion de glaucome. L’OCT de la papille a représenté 54,74 % des OCT réalisés. La GEP était bilatérale chez 284 patients et unilatérale chez 51 patients. L’âge moyen des patients était de 41.87 ans. Le rapport C/D vertical clinique moyen était de 0.52 [0.4 à 0.9]. L’épaisseur de la couche des FNR au niveau des quatre quadrants a varié de 23 à 170 μm avec une moyenne de 98.52 ± 17.22 µm. L’atteinte de la couche des FNR a été retrouvée dans 57.16% des cas. La règle de ISNT était respectée dans 47.61 % et n'était pas respectée dans 52.39 %. En cas d'atteinte des FNR, la règle ISNT était respectée dans 36.03% puis dans 63.97% des cas en l'absence d'atteinte des FNR. La surface moyenne de l'anneau neurorétinien (ANR) était de 1.63 mm2 ; la surface moyenne des excavations papillaires était de 1.43 mm2; la surface moyenne du disque optique était de 3.06 mm2 et le volume moyen de l'excavation était de 0.55 mm3. Le rapport C/D moyen à l’OCT était de 0.46 [0.21- 0.87]. Le rapport C/D vertical à l'OCT était ≥ 0.7 dans 96 yeux soit 14.33%. Conclusion. La couche des FNR est atteinte dans 57.16 % des cas à l'examen par OCT de la papille à Lomé. Ces résultats vont servir de données de base, en prenant en compte d’autres éléments, pour les études ultérieures orientées vers la recherche des étiologies des GEP.

ABSTRACT
Aim. To describe the retinal nerve fiber layer (RFNL) loss on optical coherence tomography (OCT) of the optic disc in a group of patients in Lomé. Materials and Methods: This was a descriptive and analytical prospective study conducted from 1st January to 31st December 2014 in a private ophthalmology centre that performs OCT in Lome. The variables studied were age, sex and OCT measurements of the papilla. Results: 335 examinations in 167 men and 168 women were selected. The indication of OCT of the optic disc was high cup/disc ratios (HCDR) or suspicion of glaucoma. The HCDR was bilateral in 284 patients and unilateral in 51 patients. The average age of patients was 41.87 years. The OCT of the optic disc represented 54.74% of OCTs that were realized. The clinical cup/disc (C/D) vertical ratio average was 0.52 [0.4 to 0.9]. The thickness of the RNFL at the four quadrants ranged from 23-170 µm with a mean of 98.52 ± 17.22 µm. RNFL loss was found in 57.16% of cases. The ISNT rule was respected in 47.61 % and was not respected in 52.39 %. In case of RFNL loss, the ISNT rule was respected in 36.16% and 63.02% in the case RFNL was normal. The average area of neural rim (NR) was 1.63 mm2; the average area of the cupping was 1.43 mm2; the average area of the optical disc was 3.06 mm2 and the average volume of the cupping was 0.55 mm3. The OCT overall C/D ratio average was 0.46 [0.21-0.87]. The OCT vertical C/D ratio was ≥ 0.7 in 96 eyes or 14.33%. Conclusion: RNFL loss is found in 57.16% of the OCT of the optic disc examinations in Lome. Our results will guide further studies aiming to elucidate the etiologies of HCDR in Lome.
