L’objectif de ce travail était d’évaluer les coûts de la prise en charge des AVC à l’Hôpital Général de Douala (HGD).<br />PATIENTS ET METHODES<br />Il s’agit d’une étude transversale prospective qui s’est déroulée sur une période de 12 mois allant d’Avril 2012 à Mars 2013, à l’unité de neurologie de l’HGD. Étaient inclus tous les patients âgés de plus de 15 ans, hospitalisés pour AVC confirmé par le scanner cérébral. Les patients avec hémorragie sous arachnoïdienne, thrombose veineuse cérébrale et ceux pris en charge chirurgicalement ont été exclus. Les tests de Chi2 et de Student T ont été utilisés pour comparer respectivement les variables qualitatives et les variables quantitatives.<br />RÉSULTATS<br />71 cas d’AVC ont été recensés dont 56,3% d’hommes. L’âge moyen était de 60 ±12,49 ans. Les AVC ischémiques représentaient 63,4% des cas. La durée moyenne d’hospitalisation était de 10,06 ± 9,86 jours et la durée moyenne de suivi extrahospitalier était de 9 ± 2 mois. Le coût total des AVC a été estimé à 60 075 593F CFA soit un coût moyen de 802 355 F CFA par patient. Il n’y avait pas de différence significative de coût entre les deux types d’AVC. Le coût de soin le plus onéreux était celui de la kinésithérapie.<br />CONCLUSION<br />Le coût de la prise en charge des AVC est élevé et suggère qu’une politique nationale de subvention de ces soins soit mise en place.</p><p>ABSTRACT</p><p>BACKGROUND<br />Stroke is the second leading cause of death worldwide and will constitute a major challenge for Sub-Saharan African. The aim of this study was to evaluate the cost of stroke in the Douala General Hospital in Cameroon (DGH).<br />PATIENTS AND METHODs<br />We carried out a 12 month descriptive study from 1st April 2012 to March 2013 in the neurology unit of internal medicine department of DGH. Were included patients aged above 15 years with established diagnosis of stroke by brain CT scan. Cases of subarachnoid hemorrhage, cerebral venous thrombosis and those operated for stroke were excluded. Chi square and Student T tests were used respectively to compare qualitative and quantitative variables.<br />RESULTS<br />71 strokes were recorded with 56.3% of male. t the mean age was 60 ± 12.49 years. Ischemic stroke represents 63.4%. The mean duration of hospitalization was 10.06 ± 9.86 days while the mean time of follow-up after hospitalization was 9 ± 2 months. The total cost of stroke was 60 075 583 francs CFA for a mean cost per patient of 802 355 francs CFA. There were no significant differences between the cost of ischemic and hemorrhagic stroke.
