RÉSUMÉ
Introduction et but. L’échange transfusionnel (ET) a pour objectif de réduire le pourcentage d’HbS, afin d’assurer une proportion suffisante d’hématies ayant une déformabilité normale. Ses indications sont multiples. Ce travail avait pour objectif d’en décrire les résultats et les limites chez l’enfant. Matériels et méthodes. Étude transversale rétrospective réalisée au Centre National de Référence de la Drépanocytose sur une période de 29 mois (Mars 2016- Août 2018) à partir des dossiers des patients âgés d’au plus 18 ans ayant bénéficié d’ET. La technique était manuelle : saignée, hydratation et transfusion de concentré érythrocytaire. L’indication, l’observance et l’évolution clinique ont été étudiées. Résultats. Dix patients ont été inclus. Leur âge médian était de 17 ans. Les indications étaient: accident vasculaire cérébral (3/10), priapisme (3/10), ulcère de jambe (3/10) et crise vasoocclusive sévère (1/10). Les ET étaient ponctuels chez 2/8 patients et répétés chez 8/10 patients. Chez ces derniers, le programme a été interrompu prématurément dans 6/8 cas du fait du coût élevé de la procédure. L’évolution à court terme et moyen terme a été satisfaisante chez tous les patients. Un patient est décédé huit mois après l’arrêt des ET au cours d’une récidive d’accident vasculaire cérébral. Conclusion. Les ET sont efficaces et bien tolérés. Le principal facteur limitant est leur coût élevé.

ABSTRACT
Introduction/Aim. Exchange transfusion (ET) has as objective to reduce HbS percentage, in order to obtain a sufficient proportion of red blood cells with normal shape. It has multiples indications. The aim of our work is to describe results and limitations of ET in a pediatric population. Materials and methods. This was a transversal retrospective study done at « Centre National de Référence de la Drépanocytose » within a period of 29 months (March 2016-August 2018) using patients aged 18 years or less who had received ET. The technique of ET was manual: bleeding, perfusion of saline and pack red blood cells transfusion. Indication, adherence and clinical evolution were our study variables. Results. We included ten (10) patients. Their median age was 17 years. Indications were as follows: cerebral vascular accident (3/10), priapism (3/10), leg ulcer (3/10) and severe vasoocclusive crisis (1/10). ET was done once for two patients and repeated for eight patients. In the case of repeated ET, the program was interrupted prematurely in 6/8 cases, due to the high cost of the procedure. The short and mean term evolution were satisfactory for all the patients. However, one patient died eight months after he had stopped ET in the course of recurrent stroke. Conclusion. ET is efficient and well tolerated. High cost is the main limiting factor.
