Abstract

RÉSUMÉ

OBJECTIFS. Cette étude avait pour objectif de décrire les résultats du dépistage du cancer du col par l'inspection visuelle du col après application de l'acide acétique à 5% (IVA) dans une population de Yaoundé.

METHODES. Il s'agit d'une étude transversale et descriptive menée à partir des dossiers médicaux des participantes à une campagne de dépistage du cancer du col utérin à l’Hôpital Bethesda de Yaoundé pendant la période du 11 au 15 aout 2014. Le dépistage était réalisé par l'IVA. Un frottis cervical complémentaire était réalisé si le résultat était non concluant à l'IVA. Les moyennes, les fréquences et le risque relatif étaient calculés.

RÉSULTATS. 421 femmes ont été examinées au cours de cette campagne. L’âge moyen des participantes était de 41 ± 10,6 ans. Treize (3,1%) participantes étaient séropositives au VIH (virus d'immunodéficience humaine). Vingt sept participantes (6,41%) avaient un résultat positif à l'IVA, dont 4/27(14,81%) cas étaient suspects de lésion intra épithéliale de haut grade et 23/27 (85,18%) cas suspects de lésion intra épithéliale de bas grade. Cent une (24%) participantes ayant un résultat non concluant à l'IVA avaient réalisé des frottis cervicaux complémentaires qui ont révélé 10/101 (10%) cas d'anomalies, dont 4/10 (40%) cas étaient des lésions intra épithéliales de haut grade.

CONCLUSION. La fréquence des 'anomalies à l'IVA dans cette étude est comparable à la fréquence de dysplasies cervicales rapportées dans les études antérieures au Cameroun sur le dépistage par frottis cervical.

ABSTRACT.

OBJECTIVES. This study aimed to describe the results of cervical cancer screening by visual inspection of the cervix after application of 5% acetic acid (VIA) in a population of Yaoundé.

METHODS. It was a cross sectional and descriptive study based on medical records of participants in a cancer screening campaign for cervical cancer to the Bethesda Hospital in Yaoundé during the period of 11 to 15 August 2014. The screening was performed by VIA. An additional Pap smear was performed if the result was inconclusive at the VIA. The means, frequencies and relative risks were calculated. RESULTS. 421 women were screened during this campaign. The mean women's age was 41 ± 10.6 years. Thirteen (3.1%) participants were positive for HIV (human immunodeficiency virus). Twenty-seven (6.41%) participants had a positive result to the VIA, including 4/27 (14.81%) cases which were suspected of high grade intraepithelial lesion and 23/27 (85.18%) cases which were suspected of low grade intraepithelial lesion. One hundred and one (24%) participants with an inconclusive result at the VIA had achieved additional pap smears, which revealed 10/101 (10%) cases of abnormalities, including 4/10 (40%) cases which were high grade intra epithelial lesions.

CONCLUSION. The frequency of abnormalities at the VIA in this study is similar to the frequency of cervical dysplasia reported in previous studies on the cervical cancer screening by pap smears in Cameroon.
