Abstract

RÉSUMÉ
Introduction. L'évaluation gériatrique multidimensionnelle (EGM) est définie comme un processus diagnostique interdisciplinaire multidimensionnel visant à déterminer les capacités médicales, fonctionnelles et psychosociales des patients âgés fragiles. L’objectif étant de décrire le profil gériatrique des patients âgés atteints de cancer adressés pour EGM. Méthodologie. Il s’agit d’une étude transversale rétrospective couvrant la période de Janvier 2022 à Décembre 2024, incluant les patients âgés ≥ 65 ans atteints de cancer. Les syndromes gériatriques ont été identifiés à l’aide de l’EGM. Résultats. 23 patients dont 13 femmes de médiane d’âge 77 ans(intervalle interquartile:72-82) ont été inclus. Les principales localisations de cancer étaient le sein (n=7), le poumon (n=4) et la prostate (n=3). Des métastases étaient présentes dans 100% des cas au moment de l’évaluation. La fragilité était présente chez 21 patients. Les principaux syndromes gériatriques étaient la perte d’indépendance fonctionnelle (n=17), la douleur chronique (n=11), le déclin cognitif (n=7) et la dénutrition (n=5). Concernant la décision thérapeutique, une limitation de traitement a été proposée chez 6 patients. Conclusion. La fragilité et les syndromes gériatriques sont fréquents dans ce groupe de personnes âgées atteintes de cancer. L’identification précoce des patients à risque de déclin fonctionnel pourrait être vulgarisée dans l’évaluation initiale des patients âgés atteints de cancer dans notre contexte.


ABSTRACT
Introduction. Comprehensive geriatric assessment (CGA) is a multidisciplinary model of care intended to identify older patient’s medical, functional and psychosocial limitations to develop a global plan of treatment and long-term follow-up. The objective being to describe the geriatric profile of older patients living with cancer referred for CGA. Methodology. We conducted a retrospective cross-sectional study from January 2022 to December 2023, including patients aged ≥ 65 years. The geriatric syndromes were diagnosed using the CGA model. Results. We studied 23 patients including 13 women, median age 77 years (interquartile range: 72-82) . The main cancer localizations were breast(n=7), lungs(n=4) and prostate(n=3). All of them were metastatic at the time of the assessment. Frailty was present among 21 patients. With regard to geriatric syndromes, dependency(n=17), chronic pain (n=11), cognitive decline (n=7) and malnutrition(n=5) were the most common. Limitation of treatment was proposed for 6 patients based on CGA. Conclusion. Frailty and geriatric syndromes are frequent in this group of older people with cancer. Early identification of patients at risk of functional decline could be part of the initial assessment of older patients with cancer in our setting.
