Abstract

ABSTRACT
Introduction. Digestive stomas are surgically created openings used for decompression, diversion, or nutrition that represent a globally common and essential procedure in pediatric surgery, including in the African context. The Aim was to Study the epidemiological aspects, indications, and outcomes of digestive stomas in children. Methodology. It was a five-year descriptive retrospective study carried out in the pediatric surgery department of the Mother and Child University Hospital (CHUME) of N'Djamena. The variables studied included epidemiological, clinical, and therapeutic factors. Results. Digestive stomas represented 5% of all surgeries performed during the study period, with a sex ratio of 2.3 and a mean patient age of 5.37 years. Eighty-two per cent of patients were operated on under emergency conditions and 45.67% of cases occurred during the neonatal period. Congenital pathologies constituted the leading etiology, accounting for 67.8% of cases. Colostomies comprised 71.6% of the stomas performed. All surgical procedures were conducted by senior surgeons, with a mean operative time of 32 minutes. Peristomal erythema was the most frequent complication observed. The mean interval for restoration of digestive continuity was 142 days. Conclusion. Digestive stomas are commonly performed procedures, particularly during the neonatal period for congenital conditions. The primary complications involve peristomal erythema, which can be associated with an extended recovery period.
RÉSUMÉ
Introduction. Les stomies digestives sont des ouvertures créées chirurgicalement pour la décompression, la dérivation ou la nutrition, représentant une procédure essentielle et mondialement répandue en chirurgie pédiatrique, y compris dans le contexte africain. L'objectif était d'étudier les aspects épidémiologiques, les indications et les résultats des stomies digestives chez l'enfant. Méthodologie. Il s'agissait d'une étude rétrospective descriptive de cinq ans menés dans le service de chirurgie pédiatrique du Centre Hospitalier Universitaire de la Mère et de l'Enfant (CHUME) de N'Djamena. Les variables étudiées incluaient des facteurs épidémiologiques, cliniques et thérapeutiques. Résultats. Les stomies digestives représentaient 5 % de toutes les interventions chirurgicales réalisées durant la période d'étude, avec un sex-ratio de 2,3 et un âge moyen des patients de 5,37 ans. Quatre-vingt-deux pour cent des patients ont été opérés en urgence et 45,67 % des cas concernaient la période néonatale. Les pathologies congénitales constituaient la principale étiologie, représentant 67,8 % des cas. Les colostomies représentaient 71,6 % des stomies réalisées. Toutes les procédures chirurgicales ont été effectuées par des chirurgiens seniors, avec un temps opératoire moyen de 32 minutes. L'érythème péristomial était la complication la plus fréquemment observée. Le délai moyen de rétablissement de la continuité digestive était de 142 jours. Conclusion. Les stomies digestives sont des procédures couramment pratiquées, particulièrement durant la période néonatale pour des pathologies congénitales. Les principales complications concernent l'érythème péristomial, qui peut être associé à un délai de rétablissement prolongé.
