Abstract

RÉSUMÉ
Introduction. La prise en charge orthopédique des fractures mandibulaires repose souvent sur un blocage maxillo-mandibulaire (BMM). Si les ligatures intermaxillaires sont classiques, l'adjonction d'arcs vestibulaires est une alternative. L'efficacité comparée de ces deux techniques dans des contextes comme celui du Cameroun, où la prévention des complications est cruciale, reste à préciser. Cette étude visait à comparer la stabilité occlusale et les effets iatrogènes du BMM avec arcs et ligatures versus les ligatures seules au CHU de Yaoundé. Méthodologie. Une étude quasi expérimentale a été menée de janvier à juillet 2025, incluant 26 patients adultes avec des fractures mandibulaires non condyliennes. Les patients ont été répartis en deux groupes : BMM par arcs vestibulaires d'Erich + ligatures (n=13) ou par ligatures selon Gilmer (n=13). Les critères d'évaluation principaux à 6 semaines étaient la mobilité mandibulaire et dentaire résiduelle, ainsi que l'indice de plaque (IP) comme marqueur d'hygiène et de santé parodontale. Résultats. Les groupes étaient comparables pour l'âge, le sexe et le type de fracture. Aucune mobilité mandibulaire résiduelle n'a été observée dans le groupe arcs+ligatures, contre un cas (7,7%) dans le groupe ligatures seule (p=0,749). La mobilité dentaire post-thérapeutique était identique dans les deux groupes (7,7%). En revanche, l'IP moyen à 6 semaines était significativement plus élevé dans le groupe arcs+ligatures (2,36 ± 0,54) que dans le groupe ligatures (0,97 ± 0,42 ; p<0,001), de même que la prévalence de l'inflammation gingivale (76,9% vs 7,7% ; p=0,024). Conclusion. Le BMM associant arcs et ligatures a démontré une tendance à une meilleure stabilité mandibulaire, bien que non significative sur cet échantillon limité. Cependant, il s'accompagne d'un impact négatif significatif sur l'hygiène buccale et la santé gingivale comparé à la technique par ligatures simples. Le choix technique doit donc peser ce compromis entre stabilité mécanique et préservation parodontale.
ABSTRACT
Introduction. The orthopedic management of mandibular fractures often relies on maxillomandibular fixation (MMF). While intermaxillary ligatures are standard, the addition of arch bars is an alternative. The comparative efficacy of these two techniques in settings like Cameroon, where preventing complications is crucial, requires clarification. This study aimed to compare occlusal stability and iatrogenic effects of MMF with arch bars and ligatures versus ligatures alone at the Yaoundé University Hospital. Methodology. A quasi-experimental study was conducted from January to July 2025, including 26 adult patients with non-condylar mandibular fractures. Patients were allocated to two groups: MMF using Erich arch bars + ligatures (n=13) or ligatures according to Gilmer (n=13). Primary outcomes at 6 weeks were residual mandibular and dental mobility, and the plaque index (PI) as a marker of oral hygiene and periodontal health. Results. The groups were comparable for age, sex, and fracture type. No residual mandibular mobility was observed in the arch bar+ligatures group, compared to one case (7.7%) in the ligatures-alone group (p=0.749). Post-therapeutic dental mobility was identical in both groups (7.7%). Conversely, the mean PI at 6 weeks was significantly higher in the arch bar+ligatures group (2.36 ± 0.54) than in the ligatures-alone group (0.97 ± 0.42; p<0.001), as was the prevalence of gingival inflammation (76.9% vs. 7.7%; p=0.024). Conclusion. MMF combining arch bars and ligatures demonstrated a trend towards better mandibular stability, albeit non-significant in this limited sample. However, it carries a significant negative impact on oral hygiene and gingival health compared to the simple ligature technique. The technical choice must therefore weigh this compromise between mechanical stability and periodontal preservation.
