RÉSUMÉ
Introduction. Conclusion.
ABSTRACT
Introduction. Porto-mesenteric venous thromboses are rare and severe manifestations of venous thromboembolic disease. Their clinical polymorphism, dominated by nonspecific abdominal pain, leads to diagnostic delay and potentially fatal complications such as acute mesenteric ischemia. Data on these thromboses remain limited in sub-Saharan Africa. This case series aimed to describe the clinical, etiological, therapeutic, and outcome features of four patients managed at Yalgado Ouédraogo University Hospital (Ouagadougou, Burkina Faso). Methods. This was a retrospective descriptive study of four patients admitted between November 2017 and December 2025 for porto-mesenteric venous thrombosis confirmed by CT angiography. Demographic, clinical, paraclinical, therapeutic, and outcome data were collected from medical records. Results. Mean age was 38.8 years (range 34–46), with three women and one man. Abdominal pain and nausea were present in all four patients (100%), while vomiting, fever, and constipation occurred in three (75%). Diagnostic delay ranged from five days to one month. CT angiography showed involvement of the superior mesenteric vein and portal vein in all patients, associated with mesenteric ischemia in three cases (75%) and pulmonary embolism in three cases (75%). Identified etiologies included thrombophilia (protein C and S deficiency), postpartum period, pregnancy, HIV-1 infection with rheumatoid arthritis, and prolonged bed rest. Three patients received vitamin K antagonists and one received rivaroxaban. Outcome was favorable in three patients (recovery); one patient died suddenly at five months. Conclusion. Porto-mesenteric thromboses affect young patients in Burkina Faso. Diagnostic delay remains common. Abdominal CT angiography should be performed early in any patient with persistent unexplained abdominal pain.
