La nasofibroscopie est une technique d’exploration endoscopique récente en oto-rhino-laryngologie (ORL). C’est une composante du bilan systématique de l’examen physique ORL. De réalisation facile, elle permet l’exploration par vision directe des voies aérodigestives supérieures, même chez le petit enfant et le vieillard. Peu d’études ayant été publiées à ce sujet dans notre contexte, et du fait du sous diagnostic de certaines pathologies ORL dans les pays en voie de développement, la question de savoir quelles sont les principales indications et résultats de la nasofibroscopie dans un grand service d’ORL de la ville de Douala a motivé cette étude. Objectifs. L’objectif de ce travail était de décrire les principales indications et résultats de la nasofibroscopie dans le service d’ORL/CCF de l’Hôpital Général de Douala (HGD). Matériels et méthodes. Il s’agit d’une étude transversale descriptive, avec un mode de recrutement rétrospectif et prospectif, réalisée dans le service d’ORL-CCF de l’Hôpital General de Douala, sur une période allant du 1er janvier 2014 au 30 avril 2017 ; soit 40 mois, avec une phase prospective couvrant les 4 derniers mois. Cette étude a porté sur 848 nasofibroscopies. Résultats. Au cours de l’étude, 7,4 % des consultants ont réalisé une nasofibroscopie. Les femmes ont été légèrement plus nombreuses (sex-ratio Homme/Femme de 0,93). La tranche d’âge la plus représentée a été celle de 0 à 9 ans et les enfants de moins de quatre ans représentaient 19.8% de la population d’étude. Les patients provenaient essentiellement de la ville de Douala (84,9%) et logeaient principalement dans les quartiers populaires (52%). 84% des prescripteurs était des médecins ORL. Les principales indications étaient la dysphonie (29,1%), l’obstruction nasale (27,6%) et les rhinites persistantes/rhinorrhées (22,9%). Les principaux résultats étaient les suivants : examen normal (28,9%), hypertrophie des végétations adénoïdes (21,6%), et laryngites (13,3%). La dysphonie isolée a conduit au diagnostic de laryngite dans 41,9% de cas et l’obstruction nasale a conduit dans 50% de cas à un résultat normal. Conclusion. Au service d’ORL de l’Hôpital Général de Douala, 7.4 % des consultants réalisent une nasofibroscopie. La population essentiellement jeune, réalise la nasofibroscopie principalement pour dysphonies ou obstruction nasale. L’hypertrophie des végétations adénoïdes et les laryngites sont les diagnostics les plus fréquents. La nasofibroscopie améliore la prise en charge des patients. <br />ABSTRACT<br />Introduction: Nasofibroscopy, one of the most recent endoscopic technique of exploration for Ear Nose and Throat (ENT) diseases, is a component of the systematic physical examination. Realization is easy and this technique allows diagnosis by direct visualization of the upper aerodigestive pathways even in young and elder patients. Few studies have been published on this technique in Cameroon. Many questions regarding the indications and impact of nasofibroscopy in the practice of a big ENT department of the city of Douala remain unanswered and that motivated our study. Objectives. To describe the main indications and findings of nasofibroscopy in the ENT department of the Douala General Hospital (DGH). Materials and Methods. This was a descriptive cross-sectional study with a retrospective and prospective recruitment mode conducted at the ENT unit of the DGH from 1st of January 2014 to 30th of April 2017 (40 months) with a prospective phase covering the last 3 months. Age, sex, residence of the patient, specialty of the prescribing physician clinical variables (indication, duration of symptoms), and nasofibroscopy findings were studied. Results. In our department, 7.4% of the consultants performed a nasofibroscopy. Among them, we extracted 848 nasofibroscopies for our study. The sex ratio M/F was 0.93. The most represented age group was 0 to 9 years and patients below four years represented 19.8% of the population. Patients mainly came from the city of Douala (84.9%), and lived mainly in working-class neighborhoods (52%). 84% of prescribers were ENT physicians. The most common indications were dysphonia (29.1%), nasal obstruction (27.6%) and persistent rhinitis / rhinorrhea (22.9%). The main findings were as follows: normal examination (28.9%), hypertrophy of the adenoids (21.6%), and laryngitis (13.3%). Isolated dysphonia led to the diagnosis of laryngitis in 41.9% of cases and 50% of patients presenting with nasal obstruction had a normal result. Conclusion. At the ENT Department of Douala General Hospital, nasofibroscopy is performed to 7.4% of consultation patients. Most often, these are young patients. The main indications are dysphonia and nasal obstruction.
