Il s’agissait d’identifier les facteurs épidémiologiques, cliniques, biologiques et thérapeutiques associés à la mortalité chez les patients infectés par le VIH, traités pour une tuberculose dans un service de maladies infectieuses. Patients et méthodes. Étude rétrospective et analytique qui a concerné la période de janvier 2012 à décembre 2016. Tous les dossiers exploitables des patients infectés par le VIH de 18 ans et plus, hospitalisés et traités pour une tuberculose ont été inclus. Nous avons réalisé une analyse de régression logistique pour l’identification des facteurs de risque de mortalité en hospitalisation et le seuil de significativité était de 5 %. Résultats. La prévalence de la coïnfection TB/VIH était de 8,0 %. A l’issue du séjour hospitalier, 69 patients étaient vivants (57,5 %) et 51 décédés (42,5 %). En analyse bivariée, la polypnée (p=0,020), la pleurésie (p=0,010), le stade IV OMS (p<0,001), le nombre d’infections opportunistes supérieur ou égale à deux (p=0,025), la présence de bacilles-acido-alcoolo-résistants (BAAR) dans les expectorations (p=0,049) et le diagnostic de la tuberculose chez des patients déjà sous traitement antirétroviral (ARV) (p=0,006) étaient significativement associés au décès. En analyse multivariée, la polypnée (OR ajusté= 3,15 [1,18–8,41] ; p=0,022) ; le stade IV OMS (OR ajusté= 5,31 [1,53–18,50] ; p=0,009) et la découverte de la tuberculose chez les patients déjà suivis sous antirétroviraux (OR ajusté= 4,5 [1,3–16,3] ; p=0,023) étaient indépendamment associés au décès en hospitalisation. Conclusion. La mortalité hospitalière des patients co-infectés TB/VIH était élevée. La recherche active et le traitement d’une tuberculose avant la mise sous ARV chez les patients VIH aide à réduire la mortalité précoce.</p><p>ABSTRACT<br />Objective. To identify epidemiological, clinical, biological, and therapeutic factors associated with mortality of HIV-infected patients treated for tuberculosis in an infectious disease ward. Patients and methods. This was a retrospective and analytical study covering the period from January 2012 to December 2016. All exploitable records of HIV-infected patients aged 18 years and older, hospitalized and treated for tuberculosis were included. We performed a logistic regression analysis to identify mortality risk factors during hospitalization and the threshold of significance was 5%. Results. The prevalence of TB/HIV co-infection was 8.0%. At the end of hospital stay, 69 patients were alive (57.5%) and 51 died (42.5%). In bivariate analysis, polypnea (p = 0.020), pleurisy (p = 0.010), WHO stage IV (p <0.001), number of opportunistic infections greater than or equal to two (p = 0.025), presence of acid-fast bacilli (AFB) in sputum (p = 0.049) and diagnosis of tuberculosis in patients already on antiretroviral therapy (HAART) (p = 0.006) were significantly associated to death. In multivariate analysis, polypnea (OR adjusted = 3.15 [1.18-8.41], p = 0.022); WHO stage IV (adjusted OR = 5.31 [1.53-18.50], p = 0.009) and the discovery of tuberculosis in patients already under antiretroviral therapy (OR adjusted = 4.5 [1.3- 16.3], p = 0.023) were independently associated with death in hospital. Conclusion. Hospital mortality in TB / HIV co-infected patients was high.
