L’hypertension artérielle (HTA) maligne constitue une urgence thérapeutique en raison du risque évolutif vers une encéphalopathie hypertensive, une insuffisance rénale progressive ou une défaillance cardiaque. Au Mali, il existe peu d’études réalisées sur ces problèmes d’où l’intérêt d’initier ce travail. Objectifs. déterminer la prévalence et les complications liées à l’HTA maligne dans le service de néphrologie du CHU de point “G”. Matériels et méthodes. Etude rétrospective allant du 1er janvier 2010 au 31 décembre 2014 réalisée dans le service de néphrologie et d’hémodialyse du CHU du point “G”. Etaient inclus, tous les patients insuffisants rénaux, hypertendus ou normo tendus sous traitement hospitalisés avec une rétinopathie. Résultats. Nous avons colligé 193 patients. La prévalence de l’HTA maligne était de 8,2%, avec un sexe ratio 2,5. L’âge moyen était de 37,3 ans. Il existe une relation statistiquement significative entre l’HTA grade 3 et la rétinopathie hypertensive (P=0,0006) et entre la cardiomégalie radiologique et œdème papillaire (P=0,020).  Le risque de développer l’œdème papillaire est deux fois plus élevé pour les patients avec HTA grade 3 (P=0,0002). L’OAP a été retrouvé chez sept patients (3,6%), l’encéphalopathie hypertensive a été retrouvée dans 46,6% des cas, l’IDM a été suspecté chez treize patients (6,7%), avec des signes d’ischémie myocardique à l’ECG. Le taux de mortalité spécifique était de 4,5% sur 5 ans. Conclusion. l’HTA maligne est une urgence hypertensive. Elle est devenue rare depuis l’amélioration de la prise en charge de l’HTA.<br /><br />ABSTRACT<br />Introduction. Malignant hypertension (MH) is a therapeutic emergency because of the evolving risk of hypertensive encephalopathy, progressive renal failure, or heart failure. In Mali, there are few studies carried out on these problems, hence the interest in initiating this work. Objectives. to determine the prevalence and complications of malignant hypertension in the nephrology department of point "G" University Hospital. Materials and methods. Retrospective study from 1st January 2010 to 31st December 2014 carried out in the nephrology and haemodialysis department of the "G" University Hospital Centre. Included were all patients with renal insufficiency, hypertension or normo-strained under hospital treatment with retinopathy. Results. We collected 193 patients. The prevalence of malignant hypertension was 8.2%, with a sex ratio of 2.5. The mean age was 37.3 years. There was a statistically significant relationship between MTA grade 3 and hypertensive retinopathy (P=0.0006) and between radiological cardiomegaly and papilledema (P=0.020).  The risk of developing papilledema is twice as high for patients with hypertension grade 3 (P=0.0002). PAO was found in seven patients (3.6%), hypertensive encephalopathy was found in 46.6% of cases, TIA was suspected in thirteen patients (6.7%), with signs of myocardial ischemia on the ECG. The specific mortality rate was 4.5% over 5 years. Conclusion. Malignant hypertension is a hypertensive emergency.
