L’exacerbation d’asthme est un phénomène paroxystique qui peut mettre en jeu le pronostic vital. Le but de notre étude était de décrire le profil épidémiologique et les modalités de prise en charge de l’exacerbation de l’asthme chez l’adulte. Patients et méthodes. Il s’agit d’une étude transversale descriptive rétrospective, qui a consisté à passer en revue les dossiers de 73 patients asthmatiques en exacerbation pendant une période de 12 mois au service d’aide et d’urgence (SAU) de l’HIA Omar Bongo Ondimba. Résultats. L’exacerbation d’asthme représentait 0,6% des patients enregistrés pendant la période étudiée avec une prédominance féminine (58,9%). L’âge moyen des sujets était de 24,5 ans. L’exacerbation était inaugurale dans 29,1% des cas, avec deux pics de fréquence entre les mois de Mai et Octobre. La notion d’atopie familiale était retrouvée chez 12,5% des sujets. Les comorbidités les plus fréquemment retrouvées étaient la rhinite allergique (38,1%), le reflux gastroœsophagien (20,3%), la conjonctivite allergique (17,6%), la sinusite chronique (10,1%). Les variations climatiques ; saisons sèches (17,8%) et les infections respiratoires virales (15,1%) dominaient les facteurs déclenchants des exacerbations d’asthme. Les signes de gravité étaient respiratoires (47,9%) et cardiovasculaires (43,8%). Un transfert en réanimation a été nécessaire dans 2,7% des cas. L’évolution sous traitement a été favorable dans 94,5% des cas. La mortalité a été de 1,3%. Conclusion. Les exacerbations d’asthme suivent un profil saisonnier lié aux effets environnementaux. La plupart des hospitalisations pourraient être évitées grâce à un meilleur contrôle de l’asthme et à l’amélioration de l’éducation thérapeutique des patients asthmatiques.<br /><br />ABSTRACT<br />Background. Asthma exacerbations are acute or subacute episodes, characterized by a progressive increase in one or more typical asthma symptoms that can be life-threatening. The aim of this study was to describe the epidemiology and the management of asthma exacerbations in adults. Patients and methods. It was a cross sectional retrospective and descriptive study covering a 12 months period, which consisted in reviewing the medical records of 73 asthmatic patients experiencing asthma exacerbation at the emergency service of the HIA Omar Bongo Ondimba. Results. Asthma exacerbations represented 0.6% of the total number of patients registered during the study period, with a female predominance (58.9%). The patients had an average age of 24.5 years. The exacerbation was inaugural in 29.1% of the cases with two frequency peaks between the months of May and October. Family atopy history was found in 12.5% of the cases. The most common comorbidities were allergic rhinitis (38.1%), gastroesophageal reflux (20.3%), allergic conjunctivitis (17.6%), and chronic sinusitis (10.1%). Climate change, dry seasons (17.8%), and viral respiratory infections (15.1%) were the most common triggers of asthma exacerbations. The severity signs were mainly respiratory (47.9%) and cardiovascular (43.8%). Resuscitation was required in 2.7% of the cases. The outcome with treatment was favorable for 94.5% of patients, with a mortality rate standing at 1.3%. Conclusion. Asthma exacerbations follow a seasonal pattern related to environmental effects.
