L’amygdalectomie est une intervention courante en ORL. La période postopératoire n’est pas dénuée de morbidité. L’objectif de cette étude était de relever l’efficacité des antibiotiques dans la réduction de la morbidité postopératoire. Méthodologie: Il s’agissait d’un essai clinique non randomisé mené dans le service ORL de l’Hôpital Central de Yaoundé de janvier à septembre 2014. Les candidats à une amygdalectomie, après consentement éclairé, ont été randomisés en deux groupes : ceux ayant reçu l’amoxicilline-acide clavulanique pendant sept jours après leur chirurgie (Cas) et ceux n’ayant rien reçu (Témoins). Les variables comparées étaient la fièvre, le saignement, la douleur et le temps de reprise alimentaire. L’étude avait été acceptée par le comité d é éthique de la Faculté de Médecine et des Sciences Biomédicales de l’Université de Yaoundé I. Résultats.: Cent cinq patients dont 51 du groupe des cas et 54 du groupe des témoins, ont été inclus. La fièvre était présente chez 34,3% des cas et 22,2% des témoins. Un saignement avait été noté chez 3 ,7% des témoins et 2,0% des cas, sans différence statistique (p=0,59). L’intensité et la durée de la douleur ne montraient pas de différence (p>0,05). La reprise alimentaire était effective dès le huitième jour chez 31,4% des cas contre 40,7% des témoins, sans différence significative (p=0,569). Conclusion. Cette étude suggère que l’utilisation des antibiotiques ne réduit pas la morbidité après une amygdalectomie. Leur prescription devrait se faire au cas part cas et non systématiquement, afin de protéger les patients des effets indésirables potentiels.</p> <p>ABSTRACT<br />Introduction. Tonsillectomy associated or not to adenoidectomy is one of the most frequent surgical procedures in otolaryngology. Despite significant advances in surgical and anesthesiology techniques, post-operative morbidity remains a non-negligible problem. The objective of the study was to determine if antibiotic therapy following tonsillectomy has an impact on post-operative morbidity. Methodology: It was a non randomized clinical trial carried out in the ENT (ear nose throat) units of five reference hospitals of Yaoundé during a period of nine months. The sample was divided into two groups; one consisting of patients placed on antibiotics (amoxicillin-clavulanate) and analgesics (cases) during seven days after tonsillectomy and another placed exclusively on analgesics (controls). Results. A total of 105 patients were included in the study with 51 cases and 54 controls. The mean age was 10 ± 10 years. Immediately after surgery, on the same day, 36 patients (34.3%) developed fever with a temperature between 38°C and 40°C, among which 20 (55.5%) were on antibiotics (p=0.06). The rate of bleeding was not different comparing the cases and controls (2% against 7.4% respectively, p=1.19). The pain intensity was similar in both groups of patients (p>0.05). Return to normal diet was similar in both groups with 40.7% of controls resuming normal diet on the 8th day following surgery against 31.4% of cases (p=0.569). Conclusion.
