L’insuffisance rénale aigue (IRA) est une défaillance d’organe de survenue fréquente en réanimation. L’objectif de cette étude pilote était de décrire l’EER dans le service de réanimation polyvalente du CHU du Point G. Matériels et méthode. Il s’agissait d’une étude rétrospective de juin 2019 à novembre 2020. Nous avons analysé les dossiers de tous les patients admis en réanimation pour insuffisance rénale aiguë définie selon les critères de la classification KDIGO avec recours à une EER. Résultats. Nous avons colligé 63 dossiers de patients dialysés en réanimation sur 683 admissions soit une fréquence de 9,2%. L’âge moyen était de 40,7 ± 17,4 ans. Le sexe ratio était de 0,61. Les troubles de la conscience étaient retrouvés chez 39,4% des patients, l’hypertension artérielle chez 42,9% des cas. Le choc septique (22,2%) et la néphropathie vasculaire (22,2%) étaient les étiologies les plus fréquentes. Les patients classés stade 3 de KDIGO étaient majoritaires (87,3%). L’hyperurémie (61,9%) et l’hyperkaliémie (33,3%) étaient les indications prédominantes. L’EER était initiée tardivement chez 95,2% des patients. La récupération de la fonction rénale était totale chez un patient et partielle chez 48. La durée moyenne d’hospitalisation était de 5 ± 4,1 jours (extrêmes : 1 jour et 21 jours) avec un taux de létalité était de 55,6%. Conclusion. L’insuffisance rénale aiguë reste une complication fréquente et grave en réanimation. Son délai d’initiation est tardif dans notre étude avec une pratique associée à une mortalité élevée. <br />ABSTRACT<br />Introduction. Acute renal failure (AKI) is an organ failure that frequently occurs in intensive care. The objective of this work was to describe the characteristics and prognostic factors of AKI in the multipurpose intensive care unit of the G Point CHU. Materials and method. This was a retrospective study from June 2019 to November 2020. We analyzed the files of all patients admitted to intensive care for acute renal failure defined according to the criteria of the KDIGO classification and, having benefited from an ERP. Results. We collected 63 records of dialysis patients in intensive care out of 683 admissions, ie a frequency of 9.2%. The mean age was 40.7 ± 17.4 years. Females predominated (61.9%) with a ratio of 0.61. Consciousness disturbances were found in 39.4% of patients, arterial hypertension in 42.9% of cases. Septic shock (22.2%) and vascular nephropathy (22.2%) were the most common etiologies. The majority of patients classified as stage 3 of KDIGO (87.3%). Hyperuremia (61.9%) and hyperkalemia (33.3%) were the predominant indications. The ERP was initiated late in 95.2% of patients. Renal function recovery was complete in one patient and partial in 48. The mean hospital stay was 5 ± 4.1 days (range: 1 day and 21 days). The case fatality rate was 55.6%. Conclusion. Acute renal failure remains a frequent and serious complication in intensive care.
