La césarienne, est généralement faite sous anesthésie locorégionale. Notre objectif était d’évaluer l’efficacité de l’association Bupivacaïne + Kétamine par voie péridurale au cours de la césarienne élective. Matériels et Méthodes. Nous avons mené un essai clinique non randomisé en simple aveugle dans deux hôpitaux de Yaoundé du 1er Novembre 2018 à 31 Juillet 2019. Les parturientes ont été recrutées au cours de la consultation d’anesthésie après obtention au préalable du consentement éclairé et l’autorisation des dirigeants des différents hôpitaux. La répartition était faite le jour de l’intervention en 02 groupes : Groupe Bupivacaïne + Kétamine (A) et Groupe Bupivacaïne solution physiologique seule (T). Nous avons recherché respectivement les caractéristiques des blocs, les effets indésirables et la consommation d’analgésiques en période post opératoire. Les données obtenues ont été analysés grâce au logiciel SPSS version 2.0. Résultats. Au total 44 parturientes ont été recrutés et reparties dans les deux Groupes. L’âge moyen était de 32,5 ± 5,0 ans. L’indication la plus fréquente était l’utérus cicatriciel. Le bloc sensitif s’installait au bout de11 et 10 min dans le groupe A et le groupe T, respectivement. L’hypotension artérielle était plus retrouvée dans le groupe kétamine dans 36,4 en per opératoires. La durée du bloc sensitif était de 160 min dans le groupe A et de 80 min dans le groupe T. Cette différence était statistiquement significative. Conclusion. L’utilisation de la kétamine par voie péridurale comme adjuvante entraine des effets indésirables mineurs dominés par l’hypotension artérielle souvent bien tolérés.<br />ABSTRACT<br />Objective. Caesarian section is generally done under locoregional anaesthesia. Our objective was to evaluate the efficacy of association of Bupivacaine+ ketamine by the epidural route during elective caesarian section. Material and Methods. We did a non-randomized blind trial in two hospital in Yaounde from 1st November 2018 to 31st july 2019. The parturiants were recruited during anaesthesia consultation; after informed consert and authorization form the managers of two hospitals. The repartition was done into two groups on the days of the intervention: Group A Bupivacaine+ Ketamine and Group T Bupivacaine only. We looked for the charesteristics of the bloks, the undesirable effets and the results obtained were analysed using the SPSS version 2.0 Software. Results. 44 parturientes recruited were distributed to two groups. The average age was 32,5± 5.0 years. The most frequent indication was the scarred uterus. The sensitive block was installed after 11 and 10 minutes in group A and group T respectively. Arterial Hypotension was found more in the kétamine group in 36,4% during surgery. The duration of the sensitive block was 160 minutes in group A and 80 minutes in group T. This difference was statistically significant. Conclusion.
