Il s’agissait d’étudier les aspects épidémiologiques, cliniques et pronostiques des complications métaboliques aigües du diabète. Matériels et méthodes. Il s’agit d’une étude descriptive, analytique et prospective sur une période d’un an, allant du 01er Octobre au 31 Septembre 2019 au Service d’Accueil des Urgences (SAU) du Centre Hospitalier Universitaire Gabriel Touré (CHU GT) de Bamako. Ont été inclus tous les patients diabétiques connus ou non admis dans le service pour complications métaboliques aiguës du diabète. Les données d’intêt étaient la fréquence, l’identification des facteurs de risque associés à leur survenue, la description des manifestations cliniques et la détermination des facteurs de mortalité. Résultats. Au total, 33 patients ont été colligés. Leur âge moyen était de 52,5 ± 17 ans. Le sex-ratio était de 0,5. Les patients était amenés du domicile dans 57,6% des cas et l’altération de la conscience était le motif d’admission le plus fréquent (42,4%). À l’admission, la clinique était majoritairement marquée par une polypnée, une dyspnée de Kussmaul, une déshydratation, une tachycardie et une hyperthermie. L’infection a été le facteur de décompensation chez 63,6% des patients. L’acétoacidose a été la complication majeure avec 54,5% suivie du coma hyperosmolaire avec 24,2% et du coma hypoglycémique avec 21,2%. L’évolution a été défavorable chez 09 patients soit 27,3% des cas Conclusion. Les complications métaboliques aiguës du diabète sont un réel problème de santé à Bamako à cause de leur mortalité élevée. La cétoacidose est la complication la plus fréquemment retrouvée. L’infection est la principale cause de décompensation. La surveillance clinique et biologique doit être plus stricte afin d’améliorer le pronostic.<br />ABSTRACT<br />Objectives: The aim was to report the epidemiology, clinical presentation, management and outcome patients with acute metabolic complications of diabetes. Materials and methods: This was a descriptive and prospective study over a period of one year, from October 01 to September 31, 2019 at the Reception and Emergency Department of the Centre Hospitalier Universitaire Gabriel Touré (CHU GT) in Bamako. We included all diabetic patients admitted to the department for acute metabolic complications of diabetes. Our data of interest were the frequency, the risk factors associated with their occurrence, the clinical manifestations and determinants of mortality. Results: A total of 33 patients were collected. Acute metabolic complications of diabetes are a real health issue in Bamako because of the high mortality. Ketoacidosis is the most common complication. Infection is the main cause of decompensation.
