Helicobacter pylori (H. pylori) est un problème de santé publique selon l’organisation mondiale de la santé qui estime sa prévalence à 50% dans le monde et 80% en Afrique. L’absence de données chez l’adulte au Gabon, nous a conduit à réaliser ce travail avec l’objectif de déterminer la prévalence de H.pylori au centre d’endoscopie du Centre Hospitalier Universitaire de Libreville (CHUL) et définir le profil épidémiologique et clinique du patient infecté par H. pylori. Patients et méthodes. Il s’agit d’une étude prospective et descriptive. Elle s’est déroulée entre le 1er Mars 2015 au 30 Mars 2016 au CHUL. Nous avons inclus les patients reçus en endoscopie digestive haute, ayant accepté de répondre à un questionnaire complété par le gastroentérologue. Au cours de l’endoscopie, deux biopsies antrales et deux fundiques ont été réalisées. La recherche de H. pylori était réalisée au microscope optique après coloration au Giemsa modifiée. Les patients inclus ont été répartis en 2 groupes : groupe 1 (patients H .pylori positif) et groupe 2 (patients H.pylori négatif). Résultats. Nous avons étudié 728 patients dont 426 femmes et 302 hommes, soit un sex-ratio de 0,7. L’âge moyen était de 44 ans (±16.4) avec des extrêmes de 18 et 87 ans. La recherche de H.pylori était positive chez 273 patients soit une prévalence de 37,5 %. La prévalence de H. pylori était de 40,2% avant 50 ans et 33,1% après 50 ans (p = 0,001). Il y avait 273 patients dans le groupe 1 et 455 dans le groupe 2. Le sex-ratio était respectivement de 0,7 dans le groupe 1 et 0,6 dans le groupe 2 (p=0,7). L’âge moyen était respectivement de 41,9 ans dans le groupe 1 et 45,2 ans dans le groupe 2 (p&lt;0,001). La compliance à l’alcool était respectivement de 14,3% et 14,1% (p=0,9). Les indications de l’endoscopie dans le groupe 1 étaient les manifestations du reflux gastro-œsophagien (RGO) (65,9%) l’hémorragie digestive haute (9,5%), l’hypertension portale (9,5%), l’anémie (8,4%) et autres (6,7%). Pour le groupe 2, les indications étaient les manifestations de reflux gastro-œsophagien (62,9%), l’hémorragie digestive haute (13,2%), l’hypertension portale (11,9%), l’anémie (8,4%) et autre (3,6%). La fréquence des anomalies gastriques endoscopiques était de 85,3% contre 74,3% (p&lt;0,001). Au niveau duodénal, la fréquence des anomalies endoscopiques était de 60,4% versus 51,6% (p&lt;0,001). La gastrite active et les lésions précancéreuses étaient statistiquement associées à la présence de H. pylori (p&lt;0,001). Conclusion. La prévalence de H. pylori au centre d’endoscopie du CHUL est de 37,5%. Le patient type est une adulte âgée de moins de 50 ans, se plaignant de RGO chez qui l’endoscopie haute retrouve des lésions inflammatoires gastriques et/ou duodénales.</p><p>ABSTRACT<br />Introduction. Helicobacter pylori (H.pylori) represent a public health challenges according to the World Health Organization. The prevalence is estimated around 50% in the world and 80% in Africa. The absence of adults data in Gabon led us to carry out this work with the aim of first to determining the prevalence of H. pylori and second to define the epidemiological profile and clinic of the patient infected with H.pylori. The case study took place at the endoscopy Center at Libreville University Hospital Center (CHUL). Patients and methods. This was a prospective and cross sectional descriptive study which took place between March 1st, 2015 and March 30th, 2016. We included patients who agreed to respond the questionnaire completed by the gastroenterologist. During the endoscopy, two antral biopsies and two fundal were performed. The search for H. pylori was performed with the optical microscope after staining with modified GIEMSA. The patients were divided in two groups: Group 1 (positive H. pylori patients) and Group 2 (negative H. pylori patients). Results. We studied 728 patients from 426 women and 302 men a sex ratio of 0,7. The average age was 44 years (± 16,4), with extremes of 18 and 87 years. The research for H. pylori was positive in 273 patients, giving a prevalence of 37,5%. Group 1 included 273 patients versus 455 in Group 2. The sex ratio was 0,7 in Group 1 and 0,6, respectively, in Group 2 (P = 0,7). The mean of age was 41,9 years in Group 1 and 45,2 years in Group 2 (p &lt; 0,001), respectively. The prevalence of H. pylori was higher regardless of the age group before 50 years. The prevalence of H. pylori was 40,2% before 50 years and 33,1% after 50 years (P = 0,001). Compliance to alcohol was 14,3% and 14,1% respectively (P = 0,9). Indications of endoscopy in Group 1 were manifestations of gastroesophageal reflux disease (65,9%), high digestive hemorrhage (9,5%), Portal hypertension (9,5%), anemia (8,4%) and others (6,7%). For group 2, the indications were manifestations of gastroesophageal reflux disease (62,9%), high gastrointestinal hemorrhage (13,2%), Portal hypertension (11,9%), anemia (8,4%) and other (3,6%). The frequency of endoscopic gastric anomalies was 85,3% in Group 1 versus 74,3% in Group 2 (p &lt; 0,001). At the duodenal level, the frequency of endoscopic anomalies was 60,4% in Group 1 versus 51,6% in Group 2 (p &lt; 0,001). Active gastritis and precancerous lesions were statistically associated with the presence of H. pylori (P &lt; 0,001). Conclusion. The prevalence of H. pylori at the CHUL Endoscopy Center is 37,5%. The typical patient's is a young adult, less aged 50 years or less, complaining of gastroesophageal reflux disease and in whom the endsocopy finds inflammatory gastric and/or duodenal lesions.</p> ER -
