Notre travail avait pour but de déterminer la prévalence de la drépanocytose homozygote  au cours de l’activité chirurgicale oto-rhino-laryngologique et d’en identifier les principales indications opératoires. Patients et méthodes. Nous avons réalisé une étude transversale, rétrospective de 10 ans. L’étude a inclus tous les patients atteints de drépanocytose homozygote suivis dans les services d’ORL de Brazzaville pour une pathologie chirurgicale. Les paramètres étudiés étaient la prévalence hospitalière, l’âge, le sexe, la provenance, l’indication opératoire et les taux d’hémoglobine. Résultats. Au total, 92 patients drépanocytaires homozygotes ont été recrutés représentant une prévalence hospitalière de 4,6% des patients opérés dans les services d’ORL à Brazzaville. Leur âge moyen était de 10,2 ans et le sex-rato H/F de 0,5. Chez 65% des patients, la découverte de la drépanocytose était fortuite au cours du bilan préopératoire. La principale indication opératoire était la tonsillectomie (68,4%), suivie des adénectomies (18,5%) et des kystectomies cervicales (5,4%). Chez les sujets non  référés (65% de l’effectif), le pourcentage de sujets avec un  taux d’hémoglobine inférieur à 8 g/dl était de 56.2% ; mais, dans le groupe des sujets référés, ce pourcentage descendait à 2.2 % et cette différence était statistiquement significative (P=0,02).  Conclusion. Dans les services ORL de Brazzaville, la découverte de la drépanocytose est encore souvent fortuite au cours de l’activité ORL. Il parait nécessaire de rendre systématique son dépistage en période préopératoire.<br /><br />ABSTRACT<br />Objective. To describe the prevalence of homozygous sickle cell anemia among subjects who have ENT surgery in Brazzaville and the surgical indications. Patients and methods. This was a cross-sectional descriptive, 10-year retrospective study that included all patients with homozygous sickle cell disease who were treated in ENT departments for surgical pathology in Brazzaville. Our variables of interest were prevalence of sickle cell disease among patients, age, and sex, and origin, indication of surgery and hemoglobin levels. Results. A total of 92 homozygous sickle cell patients were collected, representing a prevalence of 4.6% of all patients operated in ENT departments of Brazzaville. The average age was 10.2 years; the sex ration M/F was 0.5. For 65% of patients, sickle cell anemia was fortuitous discovered during the preoperative assessment. The main indications of surgery were tonsillectomy (68.4%) cervical adenectomies (18.5%) and cervical cystectomies (5.4%). In the group of self referred or non referred patients (65% of the population), the percentage of subjects with a significantly low hemoglobin level (&lt; 8 g/dl) was high (56.2%); on the other hand, this percentage was only 2.2 % in the group of refereed patients and the difference was statistically significant (P=0.02. Conclusion. : In the ENT departments of Brazzaville, sickle cell anemia is usually fortuitously discovered during surgical activity. So it seems necessary to make systematic screening of sickle cell anemia before any surgery is warranted.</p> ER -
