En Afrique le taux de mortalité des urgences chirurgicales abdominales reste élevé, allant jusqu’à 15%. L’objectif de ce travail était de décrire les caractéristiques des patients décédés après une chirurgie indiquée pour abdomen aigu non traumatique à l’Hôpital Central de Yaoundé (HCY). Population et Méthodes. Nous avons mené une étude transversale descriptive avec collecte prospective des données sur une période de 8 mois. Tous les patients âgés de plus de 17 ans décédés dans la phase précoce postopératoire (soit 7 jours) après une intervention pour abdomen aigue non traumatique à l’HCY ont été inclus. Les données évaluées étaient les caractéristiques sociodémographiques, clinique, opératoire et la morbi-mortalité. Résultats. Au total 188 patients ont été opérés, parmi lesquels 26 patients décédé soit une mortalité de 13,82%. Le sex ratio H/F était de 1,16. L’âge moyen des patients décédés était de 42,95±15,3 ans. Les patients décédés provenaient en majorité de la zone rurale (n=11, 42,30%). La péritonite représentait le diagnostic préopératoire le plus fréquent (n=15, 57,70%). Plus de 50% des patients ont été pris en charge plus de 24 heures après leur arrivée aux urgences. La résection intestinale avec anastomose était le geste chirurgical le plus réalisé (n=14, 53%). Le choc septique et le choc hypovolémique étaient les premières causes de décès. Conclusion. La mortalité des abdomens aigues chirurgicaux reste encore élevée et est assombri par les délais élevés de prise en charge dans notre contexte.<br />ABSTRACT<br />Objective. In Africa the mortality rate of abdominal surgical emergencies is still high, sometimes up to 15%. The aim of this study was to describe the characteristics of patients who died after surgery indicated for acute non-traumatic abdomen at the Yaoundé Central Hospital (YCH). Population and Methods. We conducted a descriptive cross-sectional study with prospective data collection over an 8-month period. All patients over 17 years of age who died in the early postoperative phase (i.e. 7 days) after an intervention for acute non-traumatic abdomen at the YCH were included. Data evaluated were socio-demographic, clinical, operative and morbidity characteristics. Results. A total of 188 patients were operated on during the study period, of whom 26 patients died, representing a mortality of 13.82%. The sex ratio M/F was 1.16. The mean age of the deceased patients was 42.95±15.3 years. The majority of the deceased patients were from rural areas (n=11, 42.30%). Peritonitis was the most frequent preoperative diagnosis (n=15, 57.70%). More than 50% of the patients who died were managed more than 24 hours after their arrival in the emergency room. Intestinal resection with anastomosis was the most common surgical procedure performed in deceased patients (n=14, 53%). Septic shock and hypovolaemic shock were the first causes of death in patients. Conclusion. The mortality of acute surgical abdomen is still high and is overshadowed by the high delay of management in our context.<br /><br /></p> ER -
