La transfusion sanguine est une pratique courante en pédiatrie, notamment en zone d’endémie palustre. C’est la méthode par excellence de prise en charge des anémies sévères. Toutefois, après transfusion, certains enfants continuent de décéder. Notre objectif était de rechercher les déterminants de ces décès afin de proposer des solutions adéquates.<br />MÉTHODOLOGIE. Nous avons réalisé une étude transversale et analytique sur des enfants âgés de 3 à 59 mois transfusés au Centre Mère et Enfant de la Fondation Chantal Biya (CME/FCB) et au Centre Hospitalier et Universitaire de Yaoundé (CHUY) du 1er janvier 2009 au 29 Février 2012. Les informations sur les patients étaient recueillies dans les registres d’hospitalisation et les dossiers médicaux des patients. Elles concernaient les caractéristiques sociodémographiques des patients, leur itinéraire thérapeutique ainsi que l’issue du traitement.<br />RÉSULTATS. Quatre cents enfants âgés de 3 mois à 5 ans ont été transfusés dont 146 venant du CHUY et 254 du CME/FCB. Cent deux enfants sont décédés après transfusion, soit un taux de décès post transfusionnel de 25,5%. Le sex-ratio était de 1,3. L’âge moyen des patients était de 24,81 ± 15,43 mois. La tranche d’âge des moins de deux ans était la plus représentée, soit 221 patients (55,30%). L’anémie microcytaire hypochrome a été retrouvée chez 55,9% d’enfants. Le paludisme était la maladie sous-jacente la plus fréquente, concernant 68,8% de patients. Les facteurs significativement associés à la mortalité post-transfusionnelle étaient un traitement inadéquat du paludisme, la malnutrition, la méningite et la transfusion de sang total.<br />CONCLUSION. La prévention et un traitement adéquat du paludisme simple et grave permettraient de réduire significativement les besoins de transfusion et le cas échéant, la mortalité post-transfusionnelle.</p><p>Abstract<br />AIM. Blood transfusion is a common practice in pediatrics, especially in malaria-endemic areas. This is the method par-excellence of management for severe anemia. However, even after transfusion, some children will die. Our objective was to investigate the determinants of these deaths in order to propose appropriate solutions.<br />METHODS. We conducted a cross-sectional and analytical study of children aged 3 to 59 months transfused at the Mother and Child Center of Chantal Biya Foundation (MCC/CBF) and Yaounde Teaching Hospital (YTH) from 1 January 2009 to 29 February 2012. The data was collected from hospital registers and the medical records of the patients. They concerned the sociodemographic characteristics of the patients, their therapeutic itineraries and treatment outcome.<br />RESULTS. Four hundred children aged 3 months to 5 years were transfused including 146 at the YTH and 254 at the MCC/CBF. One hundred and two children died after transfusion, giving a post-transfusion fatality rate of 25.5%. The sex ratio was 1.3. The average age of patients was 24.81 ± 15.43 months. The children were mostly less than two years old, with 221 patients (55.30%). Hypochromic microcytic anemia was found in 55.9% of children. Malaria was the most common underlying disease, found in about 68.8% of patients. Factors significantly associated with post-transfusion mortality were inadequate treatment of malaria, malnutrition, meningitis and transfusion of whole blood.<br />CONCLUSION. Prevention and proper treatment of uncomplicated and severe malaria would significantly reduce transfusion requirements and, where appropriate, the post-transfusion mortality.</p> ER -
