Papillary Thyroid Carcinoma on Bilateral Struma Ovarii: An Exceptional Presentation

RÉSUMÉ
Le cancer thyroïdien sur goitre ovarien est une tumeur très rare de l'ovaire. C’est un tératome mature monodermique dégénéré. Souvent de découverte fortuite, il représente 0,01 % des tumeurs ovariennes et 5 à 10 % des goitres ovariens. L’imagerie peut évoquer un tératome mais ne permet pas formellement de trancher sur son caractère malin. Le diagnostic de certitude reste histopathologique. La notion de malignité prend en compte les atypies cellulaires, le pléomorphisme nucléaire, les mitoses, l’invasion vasculaire ou de la capsule ovarienne. Le carcinome papillaire est le type histologique le plus fréquent. Les auteurs rapportent un cas de carcinome papillaire de la thyroïde sur struma ovarii avec des métastases vertébrales chez une multipare de 33 ans. Bien qu'exceptionnelles, les métastases osseuses, en particulier vertébrales, et les localisations bilatérales doivent être recherchées en présence d'un carcinome thyroïdien développé sur le goitre ovarien, ce carcinome n’a pas toujours les mêmes caractéristiques nucléaires que la lésion sur la thyroïde.
ABSTRACT
Thyroid cancer on struma ovarii is a very rare tumour of the ovary. It is a degenerated mature monodermal teratoma. Often discovered incidentally, it represents 0.01% of ovarian tumours and 5 to 10% of struma ovarii. Imaging may suggest a teratoma, but cannot formally determine whether it is malignant. The diagnosis of certainty remains histopathological. The notion of malignancy takes into account cellular atypia, nuclear pleomorphism, mitosis, vascular invasion or invasion of the ovarian capsule. Papillary carcinoma is the most common histological type. The authors report a case of papillary thyroid carcinoma on struma ovarii with vertebral metastases in a 33-year-old multiparous. Although exceptional, bone metastases, particularly vertebral metastases, and bilateral locations should be investigated in the presence of thyroid carcinoma developed on struma ovarii, this carcinoma not always has the same nuclear characteristics as lesion on the thyroid itself.
