RÉSUMÉ
Le papillome inversé est une tumeur épithéliale bénigne dont la particularité est son extension dans le stroma sous-jacent de la cavité nasale et des sinus paranasaux. Nous décrivons une présentation rare d'un papillome inversé impliquant les cavités naso-sinusiennes droites avec une extension sous-durale et orbitaire chez un patient de 47 ans venu consulter pour douleur et l'obstruction nasale. Des interventions diagnostiques et thérapeutiques ont été réalisées pour gérer ce cas de manière multidisciplinaire. Une tomodensitométrie (TDM) a révélé une masse dans le sinus maxillaire droit, soufflant la paroi intersinuso-nasale et s'étendant dans le cavum en arrière, dans le vestibule nasal en avant avec une déviation de la cloison nasale vers la gauche. Le scanner avait également montré une extension sous-durale et des signes d'invasion de l'orbite droite. Une excision chirurgicale de la masse a été réalisée par une double équipe, comprenant des oto-rhino-laryngologistes et des neurochirurgiens.
L'évolution postopératoire avait été marquée par une amélioration de l'obstruction nasale, des douleurs faciales et la persistance de la cécité.
ABSTRACT
Inverted papilloma is a benign epithelial tumor with the particularity of extending into the underlying stroma of the nasal cavity and paranasal sinuses. We describe a rare presentation of an inverted papilloma involving the right nasal sinuses with subdural and orbital extension in a 47-year-old patient who presented with nasal pain and obstruction. Diagnostic and therapeutic interventions were performed to manage this case in a multidisciplinary manner. A computed tomography (CT) scan revealed a mass in the right maxillary sinus, protruding into the intersinus-nasal wall and extending into the posterior nasal cavity, in the anterior nasal vestibule with deviation of the nasal septum to the left. The scan also showed subdural extension and signs of invasion of the right orbit. Surgical excision of the mass was performed by a dual team, including otorhinolaryngologists and neurosurgeons. The postoperative course was marked by improvement in nasal obstruction, facial pain, and persistent blindness.
