Spheno-Sellar Chordoma in a Female Elderly Diabetics : a Case Report

RÉSUMÉ Nous rapportons le cas d’une patiente de 74 ans, diabétique, hypertendue, ayant des antécédents d'AVC ischémique et d'opération pour glaucome, admise en réanimation pour un trouble de la conscience insidieux. L'examen clinique a révélé un coma modéré (score de Glasgow à 10/15) sans déficit moteur ni convulsions. Les imageries (CT et IRM) ont montré un chordome sphéno-sellaire hypervascularisé et hémorragique, avec extension extradurale aux gyri frontaux et des lésions secondaires nodulaires frontales associées à un œdème cérébral frontal. Le traitement a été palliatif, incluant la réanimation hydroélectrolytique, la réhabilitation nutritionnelle et la prévention des complications de décubitus. La patiente est décédée après 8 jours de réanimation, en raison de la détérioration neurologique et d'une détresse respiratoire hypoxique. ABSTRACT We report the case of a 74-year-old female patient with a history of diabetes, hypertension, ischemic stroke, and glaucoma surgery, who was admitted to the intensive care unit due to an insidious onset of altered consciousness. Clinical examination revealed a moderate coma (Glasgow Coma Scale score of 10/15) without any motor deficits or seizures. Imaging studies (CT and MRI) revealed a hypervascular and hemorrhagic spheno-sellar chordoma, with extradural extension to the frontal gyri and secondary nodular frontal lesions, accompanied by frontal cerebral edema. Treatment was palliative, consisting of hydro-electrolytic resuscitation, nutritional support, and prevention of pressure ulcers. The patient succumbed after 8 days of intensive care due to neurological deterioration and hypoxic respiratory failure.References
