Giant Hydronephrosis on Crossed Right Renal Ectopia Associated with Left Renal Dystopia: A Case Report

RÉSUMÉ
L’association d’une hydronéphrose géante sur ectopie rénale pelvienne et d’une dystopie sur est exceptionnelle. Nous décrivons, le cas d’un adolescent de 14 ans sans antécédent pathologique particulier découvert à l’occasion de douleur abdominale chronique. Le patient a bénéficié d’une néphrectomie. L’exploration chirurgicale a montré une hydronéphrose avec une rein occupant quasiment toute la cavité abdominale dont le contenu urinaire était de 2 litres après néphrotomie et aspiration, un double pédicule vasculaire provenant des artères iliaques et dont l’un cravatait l’uretère, expliquant l’hydronéphrose. Les suites opératoires étaient simples, et la sortie du patient faite à J7.
ABSTRACT
The association of giant hydronephrosis with pelvic renal ectopia and dystopia is exceptional. We describe the case of a 14-year-old adolescent with no significant medical history, discovered due to chronic abdominal pain. The patient underwent a nephrectomy. Surgical exploration revealed hydronephrosis with a kidney occupying almost the entire abdominal cavity, containing 2 liters of urine after nephrotomy and aspiration. A double vascular pedicle originating from the iliac arteries, one of which was constricting the ureter, was found, explaining the hydronephrosis. The postoperative course was uncomplicated, and the patient was discharged on day 7.
