Hernie Lombaire de Grynfeltt : A Propos d’un Cas

RÉSUMÉ
Les hernies lombaires peuvent se situer au niveau de deux zones de faiblesse de la paroi lombaire : le triangle lombaire inférieur de Jean-Louis Petit, et le triangle lombaire supérieur de Grynfeltt. La hernie lombaire supérieure ou hernie de Grynfeltt est une pathologie rare et représente moins de 1,5% de toutes les hernies de l’abdomen. Nous rapportons le cas d’une femme de 62 ans sans antécédents particulier qui a été reçue en consultation pour un syndrome occlusif évoluant depuis 4 jours. Elle avait un bon état général et ses paramètres vitaux étaient normaux. L’examen clinique a permis de déceler une tuméfaction lombaire molle et irréductible. Un examen TDM a été réalisée et a révélé une hernie lombaire droite supérieur de Grynfeltt. Notre observation met en lumière un mode de détection d’une hernie de Grynfeltt par un syndrome occlusif.
ABSTRACT
Lumbar hernias can be located at 2 zones of weakness of the lumbar wall: the lower lumbar triangle of Jean-Louis Petit, and the upper lumbar triangle of Grynfeltt. Upper lumbar hernia or Grynfeltt's hernia is a rare pathology and accounts for only less than 1.5% of all abdominal hernias. Herein, we report the case of a 62-year-old woman with no particular history who was seen in consultation for an occlusive syndrome evolving since 4 days. She was in good general condition and her vital parameters were normal. Clinical examination revealed a soft and irreducible lumbar swelling. A CT scan was performed and revealed right upper Grynfeltt's lumbar hernia. Our observation highlights a Grynfeltt hernia presenting by an occlusive syndrome.
