Les Agressions Traumatiques Ciblées des Organes Génitaux Externes Masculins au CHU d’Abéché

RÉSUMÉ
But. Rapporter les cas de traumatismes des organes génitaux externes (OGE) masculins secondaires aux agressions. Patients et méthodes. Il s’agissait des cas d’observation des patients admis et pris en charge pour traumatisme des OGE au CHU-A entre Janvier 2019 et Septembre 2022. Résultats. L’âge moyen des patients était de 32,16 ans (extrêmes : 26 et 47 ans). L’on a noté trois cas de traumatisme par un objet contondant, deux cas de tortures criminelles visant à amputer les OGE dont un cas de strangulation des OGE et un cas de plaie balistique. Les lésions observées étaient des plaies des corps caverneux chez deux patients, une plaie délabrante infectée du scrotum associée à une plaie des corps érectiles chez un patient, une amputation traumatique du testicule associée à une plaie de la verge chez deux patients, un cas d’œdème des OGE par strangulation et un cas d’avulsion testiculaire droite. La prise en charge était faite en fonction du bilan lésionnel. Conclusion. Les agressions visant sélectivement les organes génitaux externes sont rares à travers le monde. Cependant, elles sont souvent rencontrées dans notre contexte d’exercice. Les victimes, souvent incapables de se défendre, gardent un handicape non négligeable de ces traumatismes.
ABSTRACT
Objective. To report observed cases of traumatic injuries to male external genital organs (EGO) resulting from assaults. Patients and methods. This study presents observations from six patients treated for MEGO trauma at the Abéché University Hospital between January 2019 and September 2022. Results. The average age of the six patients was 32.16 years (range: 26 to 47 years). Three cases involved blunt object trauma, two cases were instances of criminal torture aiming to mutilate EGO, including one case of EGO strangulation, and one case of ballistic wound. Injuries observed included wounds to the cavernous bodies in two patients, an infected scrotal laceration associated with erectile body injury in one patient, traumatic testicular amputation with penile wound in two patients, one case of EGO swelling due to strangulation, and one case of right testicular avulsion. Management was tailored based on the extent of injuries. Conclusion: Selective assaults on external genital organs are rare worldwide but are encountered more frequently in our practice context. Victims, often unable to defend themselves, suffer considerable disability from these traumas.
