Abstract

RÉSUMÉ
Introduction. Pour lutter contre la carence en vitamine A, le ministère sénégalais en charge de la santé recommande la supplémentation en vitamine A (SVA) chez les enfants âgés de 6 à 59 mois. Celle-ci est biannuelle et devrait commencer juste à l’âge de six mois. L’objectif était de déterminer les facteurs associés au retard de SVA chez les enfants âgés de 6 à 11 mois. Méthodes. Cette étude transversale a été menée entre février et juillet 2020 auprès d’enfants âgés de 6 à 11 mois rencontrés au centre de santé Gaspard Kamara, Dakar. Un sondage systématique était réalisé. L’entretien avec les mères a été réalisé en face à face à l’aide d’un questionnaire électronique. L’analyse était faite avec le logiciel SPSS version 22. L’identification des facteurs associés au retard de SVA a été faite á l’aide de la régression logistique. Résultats. Parmi les enfants enquêtés, 31,75 % avaient reçu la SVA avec retard. Ce retard était significativement plus élevé chez les enfants dont les mères n’étaient pas mariées (ORa=2,07; IC95%1,06-4,20), non déclarés ou déclarés tardivement à l’état civil (ORa=7,38 ; IC95%=2,32-23,42). Il en de même pour ceux dont les mères ont affirmé n’avoir jamais entendu parler de la SVA (ORa=2,93; IC95%=1,23-6,54, et manifesté un faible niveau des connaissances sur la SVA (ORa=3,39 ; IC95%=1,48-7,80), tout comme ceux n’ayant pas reçu le Penta 3 (ORa=10,94 ; IC95%=3,39-35,33). Conclusion. Il convient de renforcer l’intégration des services de la SVA et de la vaccination de routine, de collaborer avec l’état civil, d’accroitre les connaissances des mères sur la SVA.
ABSTRACT
Introduction. To fight against vitamin A deficiency, the Senegalese ministry in charge of health recommends vitamin A supplementation (VAS) in children aged 6 to 59 months. This is biannual and should start just at six months of age. The objective of the study was to identify the factors associated with delayed VAS in children aged 6 to 11 months. Methods. This cross-sectional study was carried out between February and July 2020 among children aged 6 to 11 months seen at the Gaspard Kamara health center, Dakar. A systematic sampling was carried out. The interview with mothers was conducted face to face using an electronic questionnaire. Analysis was done with SPSS version 22 software. Factors associated with the delay in VAS were identified using logistic regression. Results. Among the children surveyed, 31.75% had received VAS late. The delay was significantly greater in children whose mothers were not married (AOR = 2.07; 95% CI 1.06-4.20), not declared or declared late in the civil registry (AOR = 7, 38; 95% CI = 2.32-23.42). The same was true for those whose mothers said they had never heard of VAS (AOR = 2.93; 95% CI = 1.23-6.54, and showed a low level of knowledge about VAS (AOR = 3.39; 95% CI = 1.48-7.80), just like those who did not receive Penta 3 (AOR = 10.94; 95% CI = 3.39-35.33). Conclusions. There is a need to strengthen the integration of VAS and routine immunization services, to collaborate with the civil registry, to increase the knowledge of mothers about VAS.
