Abstract

RÉSUMÉ
Objectif. Les otomycoses sont des infections fongiques, le plus souvent chroniques et bénignes, avant tout du conduit auditif externe, rarement de l’oreille moyenne. Notre étude avait pour but d’évaluer notre conduite diagnostique et thérapeutique devant une otomycose. Méthodologie. Il s’agit d’une étude prospective de type descriptif d’une durée de six (6) mois allant du 1er Mai 2014 au 31 Octobre 2014 dans le service d’ORL et de chirurgie cervico-faciale au CHU de Donka. 27 patients ont été recrutés. Résultats. La fréquence hospitalière était 1,03 %. La moyenne d’âge des patients était de 35,24 ans avec un écart type de 4,6. Les patients de 30-39 ans représentaient 44,44% du total des patients. Le sex-ratio H/F était de 1.24. Les signes cliniques les plus communs étaient l’otorrhée purulente avec 25.92% suivie du dépôt noirâtre ou blanchâtre dans le conduit 14.81%. Le nettoyer du conduit avec le coton tige (66.66%) et les bains intempestifs (55.55%étaient) les facteurs favorisants les plus retrouvés. Le traitement a comporté à l’utilisation des antifongiques et des antihistaminiques. Conclusion. : malgré un traitement à base des antifongiques l’évolution de ces infections est imprévisible à cause de la multiplicité des facteurs favorisants.
ABSTRACT
Aim. Otomycoses are fungal infections, most often chronic and benign, primarily of the external ear canal, rarely of the middle ear. The aim of this study is to describe our diagnostic and therapeutic behavior in the face of otomycosis and to review the literature on current data. Methods. This was a prospective, descriptive study of six (6) months duration from 1 May 2014 to 31 October 2014 in the ENT and head and neck surgery department at Donka University Hospital. Results. 27 patients were recruited. The hospital prevalence was 1.03%. The average age of the patients was 35.24 years (Standard deviation: 4.6). Patients aged 30-39 years represented 44.44% of the population. The sex ratio H/F was 1.24. The most common clinical signs were purulent otorrhea (25.92%° and blackish or whitish deposition in the external auditory canal (14.81%). Cleaning the duct with cotton swab (66.66%) and inappropriate bathing (55.55%). were the most common causative factors. Treatment associated antifungals and antihistamines. Conclusion. Despite antifungal treatment, the evolution of these infections is unpredictable because of the multiplicity of the factors favoring them.
