Abstract

We show in this report titled, Pseudo-neoplasic abdominal abscess in a HIV positive patient under therapy, the interest of laparotomy in the diagnosis and treatment of subhepatic lesions of undetermined nature following CT-scan. A 47 years old HIV seropositive male patient under antiretroviral therapy, presented with subacute

epigastric pain and severe weight loss since three months. A CT-scan showed a hypodense lesion under the left lobe of the liver, interpreted as a possible pancreatitis, an abscess, a cyst or pseudocyst  or even a lymphomatous tumour. A laparotomy showed an abscess located in a cavity with a thick fibrous wall, due to an Enterobacter bacillus, sensitive to amikacine. The patient was treated with drainage of the abscess and antibiotherapy with a good result and no recurrence three years after the treatment.


 Le but de cette présentation était de relever l’intérêt de la laparotomie dans le diagnostic et le traitement des lésions soushépatiques de nature indéterminée par les examens imageriques ; il s’agit du cas d’un patient de x ans , séropositif au virus de l’immunodéficience humaine, sous tri-thérapie depuis  y ans, et présentant depuis trois mois des douleurs épigastriques subauguës avec perte de poids marquée . La tomodensitométrie avait montré une  plage hypodense de la région  pancréatique évoquant une pancréatite, un abcès , un pseudokyste ou une lésion néoplasique ; la laparotomie permit d’exclure le diagnostic de tumeur maligne et de découvrir un abcès à Enterobacter dans une coque fibroscléreuse ; le drainage de l’abcès et un traitement antibiotique avec l’amikacine  furent suivis d’une  régression de tous les signes cliniques initiaux chez un patient revu en bon état après trois ans
