Abstract
RÉSUMÉ
Objectif. Déterminer la fréquence des fractures ostéoporotiques au cours de la polyarthrite rhumatoïde au centre hospitalier de Meaux et identifier les facteurs associés aux fractures vertébrales. Matériels et méthodes. Étude transversale rétrospective et analytique menée au service de rhumatologie du centre hospitalier de Meaux de Janvier 1998 à Décembre 2018, sur 32 patients souffrant de polyarthrite rhumatoïde diagnostiquée sur la base des critères ACR 1987 et ACR/EULAR 2010 avec au moins une fracture ostéoporotique. Résultats. L’effectif comportait 27 femmes et 11 hommes d’un âge moyen de 65,9 ans.. La fréquence hospitalière des fractures ostéoporotiques au cours de la polyarthrite rhumatoïde était de 17,59%. La durée moyenne d’évolution de la maladie au moment de la fracture était de 9,68+/-7ans. Une chute était la circonstance de survenue principale (18 cas/19). Vingt-huit patients souffraient d’une douleur rachidienne: dorsolombaire (15 cas) et osseuse périphérique (13 cas). La polyarthrite rhumatoïde était déformante (13 cas), érosive (26 cas) avec des facteurs rhumatoïdes et des anticorps anti-CCP positifs respectivement dans 31 et 28 cas. Elle avait une activité faible (18 cas), modérée (12 cas), forte (1 cas) et était en rémission dans 7 cas. Les fractures étaient uniques (29 cas) et majoritairement vertébrales (25 cas). On notait une ostéoporose densitométrique dans la majorité des cas (31 cas). Au plan analytique, une ostéoporose densitométrique était significativement associée à la fracture vertébrale (P=0,01). Conclusion. Les fractures ostéoporotiques sont fréquentes au cours de la polyarthrite rhumatoïde à Meaux. Le seul facteur associé aux fractures vertébrales était l’ostéoporose densitométrique.
ABSTRACT
Objective. To determine the frequency of osteoporotic fractures in rheumatoid arthritis at the hospital center in Meaux and identify the factors associated with vertebral fractures. Materials and methods. Retrospective and analytical study conducted at rheumatology department of hospital center of Meaux from January 1998 to December 2018, relating to 32 patients suffering from rheumatoid arthritis diagnosed on the basis of ACR 1987 and ACR/EULAR 2010 criteria, with at least one osteoporotic fracture. Results. Our sample population consisted of 27 women and 11 men with an average age of 65.9 years. The hospital prevalance of osteoporotic fractures in rheumatoid arthritis was 17.59%. The mean duration of the disease at the time of the fracture was 9.68+/-7 years. A fall was the main occurrence (18 cases/19). Twenty-eight patients suffered of spinal pain, thoracolumbar (15 cases) and peripheral bone (13 cases). Rheumatoid arthritis was deforming (13 cases), erosive (26 cases) with a positivity of rheumatoid factors (31 cases) and anti-CCP antibodies (28 cases). The activity of disease was weak (18 cases), moderate (12 cases), high (1 case) and the disease was in remission (7 cases). The fractures were unique (29 cases) and predominantly vertebral (25 cases). Densitometric osteoporosis was noted in the majority of cases (31 cases). Only osteoporosis in bone density test was significantly associated with the vertebral fracture (P = 0.01). Conclusion. Osteoporotic fractures are quite common in rheumatoid arthritis in Meaux. The only factor associated with vertebral fractures is osteoporosis diagnosed by osteodensitometry.
