Abstract


RÉSUMÉ
Objectif. Décrire la prise en charge anesthésique de la césarienne d’urgence pour pré-éclampsie sévère au CHU de Brazzaville. Patientes et méthodes. Il s’est agi d’une étude descriptive, rétrospective, réalisée au bloc opératoire des urgences du CHU de Brazzaville sur une période de 12 mois (de janvier à décembre 2016). Toutes les parturientes ayant bénéficié d’une césarienne d’urgence pour pré-éclampsie sévère ont été incluses. Les variables étudiées étaient: âge, terme de la grossesse, indication de la césarienne, classe ASA, score de Mallampati, antibioprophylaxie, administration du sulfate de magnésium, type d’anesthésie, agents anesthésiques utilisés, devenir du fœtus, complications et évènements peropératoires. Résultats. L’âge moyen des parturientes était de 25,6 ±7,1 ans. La césarienne était indiquée pour éclampsie dans 50% des cas. L’anesthésie était générale dans 66,7% des cas, locorégionale dans 33,3% des cas. Le thiopental a été utilisé dans 79,2%. Le mélange bupivacaine+fentanyl a été utilisé pour toutes les rachianesthésies. Le taux de complications et évènements peropératoires enregistrés était de 41,7%. L’hypotension artérielle était représentée dans 81,8% des cas. La mortalité périnatale était de 9,7%. Nous n’avons eu aucun cas de décès maternel. Conclusion. L’éclampsie est l’indication la plus fréquente dans notre population, et l’anesthésie générale la technique la plus utilisée. Les complications peropératoires sont nombreuses et la mortalité périnatale élevée.


ABSTRACT
Objective. To describe the anaesthetic management of emergency caesarean sections for severe pre-eclampsia at the University Hospital of Brazzaville. Patients and methods. This was a descriptive, retrospective study carried out in the emergency operating room of the University Hospital of Brazzaville, over a period of 12 months (January to December 2016). All parturients who underwent an emergency caesarean section for severe pre-eclampsia were included. Variables studied were: patient age, term of pregnancy, indications for caesarean section, ASA class, Mallampati score, antibiotic prophylaxis, administration of magnesium sulphate, type of anaesthesia, anaesthetic agents used, fetal outcome, complications and intraoperative events. Results. The average age of the parturients was 25.6 ±7.1 years. Caesarean section was indicated for eclampsia in 50% of cases. General anaesthesia was used in 66.7% of cases and spinal anaesthesia in 33.3% of cases. Thiopental was used in 79.2%. Bupivacaine+fentanyl was used for all spinal anaesthesias. The rate of intraoperative complications and events recorded was 41.7%. Arterial hypotension was present in 81.8% of cases. Perinatal mortality was 9.7%. There were no cases of maternal death. Conclusion. Eclampsia is the most frequent indication in our population, and general anaesthesia is the most commonly used technique. Intraoperative complications are numerous and perinatal mortality is high.
