But. Rapporter les aspects diagnostiques et discuter les modalités de prise en charge chirurgicale face au lymphangiome kystique parotidien exofacial de l’enfant. Observations. Nous rapportons deux cas cliniques de lymphangiome kystique intra-parotidien exofacial. Les enfants ont été reçus, traités et suivis dans deux hôpitaux universitaires de Cotonou. La première observation est celle d’un garçon de 2 ans, suivi depuis la naissance pour une tuméfaction latéro-cervicale gauche. L’échographie a évoqué un lymphangiome kystique intra-parotidien confiné au lobe exofacial. Une parotidectomie exofaciale gauche a été réalisée. L’examen histo-pathologique de la pièce opératoire a confirmé le diagnostic de lymphangiome kystique intra-parotidien. Les suites opératoires ont été simples. L’évolution post-thérapeutique a été favorable avec un recul de quatre ans. La deuxième observation est celle d’une fille de dix ans suivie pour une tuméfaction de la région parotidienne droite. L’échographie cervicale a mis en évidence une poche kystique aux dépens du lobe externe de la glande parotide. Une parotidectomie exofaciale droite a été effectuée. L’examen anatomopathologique de la pièce opératoire a objectivé un lymphangiome kystique intra-parotidien. Les suites opératoires ont été simples. Il n’y a pas eu de récidive après quatre ans de suivi post-thérapeutique. Conclusion. Le lymphangiome kystique intra-parotidien de l’enfant est une malformation rare. Lorsqu’il est confiné au lobe exofacial, une parotidectomie partielle est possible avec de bons résultats. Toutefois le risque de récidive impose un suivi au long court.
RÉSUMÉ
Objective. To report the diagnostic aspects and discuss the surgical management modalities for exofacial parotid cystic lymphangioma in children. Case presentation. We report two clinical cases of exofacial intra-parotid cystic lymphangiomas. The children were received, treated and followed in two university hospitals in Cotonou. The first case is that of a 2-year-old boy, followed from birth for left lateral cervical swelling. The ultrasound suggested an intra-parotid cystic lymphangioma confined to the exofacial lobe. A left exofacial parotidectomy was performed. Histopathological examination of the operative specimen confirmed the diagnosis of intra-parotid cystic lymphangioma. The postoperative follow-up was straightforward. The post-treatment course was favorable with a follow-up of four years. The second case is that of a ten-year-old girl followed for swelling in the right parotid region. The cervical ultrasound showed a cystic pocket at the expense of the outer lobe of the parotid gland. A right exofacial parotidectomy was performed. Pathological examination of the surgical specimen revealed an intra-parotid cystic lymphangioma. The postoperative follow-up was straightforward. There was no recurrence after four years of post-treatment follow-up. Conclusion. Childhood intraparotid cystic lymphangioma is a rare malformation. When confined to the exofacial lobe, partial parotidectomy is possible with good results. However, the risk of recurrence requires long-term follow-up.
