Lymphome Non Hodgkinien Testiculaire Droit de Type Lymphoblastique chez un Adolescent: À Propos d’un Cas

ABSTRACT
Les tumeurs malignes du testicule sont dans 98 % des cas des tumeurs germinales. Les lymphomes malins testiculaires non hodgkiniens sont très rares et représentent environ 1% des tumeurs malignes testiculaires. Ce sont des tumeurs malignes testiculaires de l’homme de plus de 60 ans. Nous rapportons le cas d’un Lymphome non hodgkinien testiculaire droit de type lymphoblastique chez un patient de 15 ans de mauvaise évolution. Comme antécédent, on note une chirurgie testiculaire droite par voie inguinale non documentée réalisée deux ans plutôt. Il vient consulter pour tuméfaction testiculaire droite non douloureuse évoluant depuis environ six mois, asthénie et amaigrissement. Son examen clinique initiale retrouvait une altération de l’état général stade II OMS, un contact lombaire droit ; une masse testiculaire droite (Chevassu +) et, une poly-adénopathie inguinale bilatérale. L’histologie de l’adénectomie superficielle inguinale droite a objectivé un Lymphome non hodgkinien de type lymphoblastique au stade disséminé. Adressé au Service d’Oncologie Médicale, l’évolution a été marquée par le décès au 4ème jour après la première cure de chimiothérapie. Malgré l’extrême rareté de ce type cancer chez des patients jeunes, il doit toujours être évoqué et pris en charge selon les recommandations actuelles.

RÉSUMÉ
Malignant tumours of the testicle are in 98% of cases germinal tumours. Non-Hodgkin's testicular malignant lymphomas are very rare and represent about 1% of testicular malignant tumours. They are malignant testicular tumours in men over 60 years of age. We report the case of a right testicular non-Hodgkin's lymphoblastic lymphoma in a 15 year old patient with a poor evolution. He had a history of undocumented right testicular surgery performed two years earlier. He came to the clinic with a painless right testicular swelling that had been evolving for about six months, asthenia and weight loss. The initial clinical evaluation revealed an alteration of the general state, WHO stage II, a right lumbar contact, a right testicular mass (Chevassu +) and a bilateral inguinal polyadenopathy. The histology of the right inguinal superficial adenectomy showed a non-Hodgkin's lymphoma of lymphoblastic type at disseminated stage.Reffered to the medical oncology department, the evolution was resolved by death on 4th day after first course of chemotherapy.Despite the extreme rarity of this type of cancer in young patients, it should always be considered and managed according to current recommendations.
