RÉSUMÉ  
Le léiomyosarcome est composé de cellules à différenciation musculaire lisse et est le deuxième type histologique de sarcome rétropéritonéal le plus fréquent après le liposarcome. Connu pour son pronostic sombre, la néphrectomie élargie reste le gold standard pour les formes localisées. Nous rapportons un cas de léiomyosarcome rénal traité au service d’urologie de l’hôpital principal de Dakar. Il s’agissait d’une patiente de 55 ans qui a subi une exérèse d’une volumineuse masse lombaire gauche. L’aspect histologique et immunohistochimique était celui d’un léiomyosarcome du rein de  grade II selon FNCLCC. Le bilan d’extension était négatif. Quatre ans après l’opération, la patiente était vivante et bien portante sans signe de récidive locale ou de métastase.
ABSTRACT
Leiomyosarcoma (LMS) comprises smooth muscle differentiating cells and is the second most common type of retroperitoneal sarcoma after liposarcoma. The prognosis of LMS is generally poor and extended nephrectomy is the gold standard surgical modality for localized tumor. We report one case of renal LMS that was treated at the urology department of the Hospital Principal in Dakar. The 55-year-old female patient had extended monobloc removal of the tumor. histological and immunohistochemical analysis showed a grade II renal  leiomyosarcoma (FNCLCC). Extension workup was negative; Four years later, the patient was alive with no sign of local recurrence or distance metastasis.
