Résumé :

Introduction :

Les accidents thromboemboliques sont une complication grave de la fibrillation auriculaire que visent  à prévenir les antivitamines K(AVK). Mais ceux-ci exposent à un risque hémorragique justifiant donc une surveillance rigoureuse. Le but de notre travail était de décrire cette surveillance chez des patients à Yaoundé
Méthodologie :

Nous avons mené une étude de cohorte prospective dans les services de cardiologie de l’Hôpital Central et de l’Hôpital Général de Yaoundé. Nous y avons recruté des patients âgés d’au moins 18 ans, en fibrillation auriculaire (FA) et sous AVK et les avons suivis de Novembre 2013 à Avril 2014. Ils étaient revus au moins 1 fois après le 1er contact. Nous avons ensuite évalué le risque thromboembolique et hémorragique par les scores CHA2DS2-VASc et HAS-BLED.

Le temps passé dans la zone thérapeutique cible a été évalué par l’interpolation linéaire de Rosendaal.
Résultats :

Les 27 sujets recrutés étaient âgés en moyenne de  69 ±13,9 ans. Le sex ratio H/F était de 0,56.

Le score CHA2DS2-VASc moyen était de 4,3±1,6 et le score HAS-BLED moyen de 1,6 ±1. L’acénocoumarol était l’AVK la plus utilisée. Nous avons retrouvé 38,7% d’INR des patients compris entre 2 et 3. Nos patients passaient en moyenne 48,37% de leur temps de suivi dans l’intervalle cible d’INR. L’adaptation des posologies n’était pas faite pour 50% des INR<2, et pour 55,6% des INR compris entre 3 et 5
Conclusion :

La surveillance du traitement par AVK dans notre contexte n’est pas optimale et l’adaptation des doses de ces anticoagulants en fonction de l’INR n’est pas toujours appropriée.

 

 

Abstract:

Introduction:

Thronboembolic events are a severe complication of atrial fibrillation prevented by vitamin K antagonists. On the other hand, vitamin K antagonists expose to an increased risk of bleeding which warrants close monitoring. The aim of our study was to describe monitoring of a group of patients in Yaounde.

Methodology:                

We carried out a prospective cohort study at the cardiology units of the Central and General Hospitals in Yaounde. Patients of at least 18 years of age, having atrial fibrillation and on vitamin K antagonists were recruited and followed up from November 2013 to April 2014. Patients were seen at least once later on.  The thromboembolic and bleeding risks were evaluated using the following scores CHA2DS2-VASc and HAS-BLED respectively. The Rosendaal linear interpolation was used to evaluate the time spent within the targeted therapeutic range.

Results:

We recruited 27 subjects with a mean age of 69±13.9 years. The sex ratio M/F was 0.56. The mean for the various scores used were: CHA2DS2-VASc 4.3±1.6 and HAS-BLED 1.6±1. The most prescribed vitamin K antagonist was acenocoumarol. 38.7% of patients had INRs between 2 and 3. Patients spent 48.37% of the time within the targeted INR range during the study period. Treatment was not modified in 50% of patients who had INRs <2, and in 55.6% of patients who had INRs between 3 and 5.

Conclusion:

Monitoring of patients on vitamin K antagonist in our setting is suboptimal and adjustments made on the doses with respect to the INRs are not always justified.
