Abstract

RÉSUMÉ
Le syndrome de Charles Bonnet (SCB) est défini par la présence d’hallucinations visuelles, complexes et structurées. Il est présent chez des sujets âgés non psychotiques ayant des fonctions cognitives conservées et présentant une déficience visuelle. La spéculation existe toujours préoccupant la vraie physiopathologie d’hallucinations visuelles dans le cadre du SCB, et les traitements médicaux ne sont pas bien établis, car ne trouvant pas de consensus. Au Sénégal, les données sont encore inexistantes et l’élaboration du diagnostic reste hésitante. C’est dans ce contexte que nous nous sommes proposés de mener cette étude de cas chez un patient de 76 ans présentant des hallucinations visuelles survenues sur un terrain de cécité bilatérale associé à un glaucome de stade terminal. Le but est d’identifier les aspects cliniques et psychopathologiques permettant d’évoquer le diagnostic du SCB et d’élaborer des stratégies thérapeutiques adaptées.
ABSTRACT
Charles Bonnet Syndrome (CBS) is defined by the presence of complex, structured visual hallucinations. It is present in non-psychotic elderly subjects with retained cognitive function and visual impairment. Speculation still exists regarding the true pathophysiology of visual hallucinations in DBS, and medical treatments are not well established, as there is no consensus. In Senegal, data are still lacking, and diagnostic elaboration remains hesitant. It is in this context that we proposed to carry out this case study on a 76-year-old patient presenting with visual hallucinations in the context of bilateral blindness associated with end-stage glaucoma. The aim was to identify the clinical and psychopathological aspects that would allow us to evoke the diagnosis of CBS and to develop appropriate therapeutic strategies.
