Cardiomyopathy with Non-Compaction of the Left Ventricle: a Concomitant Complication of Venous Thromboembolic Disease in a 22-Year-Old Patient

ARÉSUMÉ
La maladie thromboembolique veineuse est un problème de santé grave qui peut survenir chez des individus de tout âge. Dans ce contexte, nous présentons le cas d'un patient âgé de 22 ans ayant développé trois épisodes de maladie thromboembolique veineuse, dont un épisode d’embolie pulmonaire. Le bilan étiologique a révélé un déficit en protéine S avec un anticorps anticardiolipine positif. Par ailleurs, une dilatation ventriculaire gauche a été fortuitement découverte lors d'un scanner thoracique, révélant une cardiomyopathie sur non-compaction du ventricule gauche. Ce cas souligne l'importance de considérer un large éventail de causes potentielles lors de l'évaluation des patients présentant des épisodes de maladie thromboembolique veineuse. Les découvertes fortuites, telles que la cardiomyopathie sur non-compaction du ventricule gauche, mettent en lumière la complexité des pathologies sous-jacentes affectant un jeune patient. Il est essentiel de mener une évaluation approfondie pour déterminer la prise en charge optimale de telles affections multifactorielles.
ABSTRACT
Venous thromboembolic disease is a serious health issue that can occur in individuals of any age. We report the case of a 22-year-old patient who developed three episodes of venous thromboembolic disease, including one episode of pulmonary embolism. The etiologic workup revealed a protein S deficiency with a positive anticardiolipin antibody. On the other hand, left ventricular dilation was incidentally discovered during a thoracic CT, revealing cardiomyopathy on non-compaction of the left ventricle. This case highlights the importance of considering a wide range of potential causes when assessing patients with episodes of venous thromboembolic disease. Incidental discoveries, such as left ventricular cardiomyopathy over non-compaction, highlight the complexity of the underlying pathologies affecting a young patient. A thorough evaluation is essential to determine the optimal management of such multifactorial conditions.
