Spontaneous Bilateral Tubal Pregnancy: A Case Report and Literature Review

RÉSUMÉ
La grossesse extra-utérine bilatérale de localisation tubaire est rare. Nous rapportons le cas d’une patiente de 28 ans prise en charge au Grand Hôpital de l’Est Francilien de Meaux venue pour douleurs pelviennes sur aménorrhée gravidique avec mauvaise cinétique du taux de B-Hcg plasmatique quantitatif. Une échographie réalisée avait objectivé une masse latéro-utérine droite et la découverte de la grossesse tubaire bilatérale a été faite en peropératoire. Le diagnostic d’une grossesse extra-utérine bilatérale est difficile car il n’existe par une présentation clinique spécifique et l’exploration de la trompe controlatérale doit être systématique lors d’une prise en charge chirurgicale d’une grossesse extra-utérine dite unilatérale.
ABSTRACT
Bilateral tubal ectopic pregnancy is rare. We report the case of a 28-year-old patient managed at the Grand Hôpital de l’Est Francilien in Meaux, who presented with pelvic pain and gestational amenorrhea, along with abnormal kinetics of quantitative plasma beta-hCG levels. An ultrasound revealed a right lateral uterine mass, and the diagnosis of bilateral tubal pregnancy was made intraoperatively. Diagnosing bilateral ectopic pregnancy is challenging due to the lack of specific clinical presentation, and exploration of the contralateral fallopian tube should be systematic during surgical management of a presumed unilateral ectopic pregnancy.
