Twenty-year abdominal textiloma after appendicectomy: a case report with literature review

RÉSUMÉ Le textilome se définit comme la rétention involontaire d’une compresse ou d’un champ dans le foyer opératoire. Malgré sa faible fréquence, les difficultés diagnostiques et l’impact économique font du textilome une pathologie dont les formes cliniques et les options thérapeutiques devraient être connues et les stratégies de prévention revisitées. Nous présentons le cas d’une femme de 40 ans, opérée 20 ans auparavant pour appendicite aigue abcédée qui a été reçue par occlusion intestinale et masse abdominale. La laparotomie a trouvé une grande compresse encapsulée dans une enveloppe épaisse et fibreuse. L’extraction s’est compliquée d’une perforation d’anse pour laquelle une résection / anastomose intestinale a été effectuée. Les suites opératoires ont été simples. Abstract Textiloma refers to the involuntary retention of a gauze or drape in the surgical site. Although infrequent, the diagnostic challenge and the economic and psychologic impact of textilome make it a disease whose clinical forms and therapeutic options should be known of every surgeon and the prevention strategies revisited. We report the case of a 40-year-old woman, operated 20 years before for acute appendicitis who was admitted for bowel occlusion and abdominal mass. Laparotomy found a large compress encapsulated in a thick, fibrous envelope. The extraction was complicated by a loop perforation for which a resection and anastomosis was performed.
