Abstract

RÉSUMÉ
Introduction. L’évaluation de la fonction ventriculaire est fondamentale pour le diagnostic de l’insuffisance cardiaque. Notre travail avait pour objectif d’évaluer la fonction ventriculaire gauche dans un groupe de patients ayant subi un scanner à Yaoundé. Méthodologie. Nous avons mené une étude transversale descriptive avec collecte rétrospective des données de novembre 2016 à juillet 2019 au service de radiologie du Centre Médical la Cathédrale de Yaoundé. Nous avons inclus tous les dossiers de coroscanners ayant au moins une phase en systole et en diastole. Nous avons exclu de l’étude les dossiers avec arythmie cardiaque. La fonction ventriculaire gauche a été évaluée par la méthode volumique directe. Les résultats ont été exprimés en moyenne ± écart type. Le coefficient de Kappa de Cohen et des intervalles de confiance de la mesure de la fraction d’éjection du ventricule gauche ont permis d’évaluer la reproductibilité inter observateur. Résultats. Le volume télédiastolique moyen du ventricule gauche était de 102,76 ± 33,48 ml/m2 et le volume télé-systolique de 38,09 ± 16,60 ml/m2. La valeur moyenne de la fraction d’éjection systolique du ventricule gauche était de 55,93 ± 14,36%. La dysfonction ventriculaire gauche était présente chez 22% des patients. Nous avons retrouvé une bonne concordance inter observateur de la fraction d’éjection du ventricule gauche avec un coefficient de Kappa de 0,63. Conclusion. La dysfonction ventriculaire gauche était présente chez un quart de la population d’étude avec une bonne concordance inter observateur de la mesure de la fraction d’éjection du ventricule gauche.
ABSTRACT
Introduction. Assessment of ventricular function is paramount for the diagnosis of heart failure. objective of this study was to evaluate the left ventricular function in Yaounde using the CT scan. Methods. We carried out a retropective cross sectional and describtive study on data collected from November 2016 to July 2019 at the medical imaging service of Centre Medical la Cathedrale of Yaounde. We included all cardiac CT scan patient files with at least one systolic and diastolic phase. We excluded all the files of patients with arrhythmias. Left ventricular function was evaluated using direct volume measurement. Results were presented with means and standard deviations. The Kappa coefficient of Cohen with confidence intervals for measurements of left ventricular ejection fraction by two observers evaluated inter-observer reproducibility. Results. The mean telediastolic volume of the left ventricle was 102,76 ± 33,48 ml/m2 and the mean telesystolic volume 38,09 ± 16,60 ml/m2. The mean left ventricular systolic ejection fraction 55,93 ± 14,36%. Left ventricle dysfunction was found in 22% of the patients. We found an inter-observer concordance regarding left ventricular ejection fraction measurement with a Kappa coefficient of 0.63. Conclusion. One quarter of the study population showed Left ventricular dysfunction. We observed a good concordance in the assessment of left ventricular ejection fraction.
